目的：探讨酒石酸布托啡诺在晚期癌症患者自控镇痛中的应用效果。方法选择晚期癌症伴疼痛的患者80例，根据治疗方式的不同将患者分为吗啡肌肉注射组（n=38）和酒石酸布托啡诺喷剂喷鼻组（n=42）。观察两组患者的治疗效果，对两组患者的不良反应发生情况、睡眠质量和治疗满意度进行比较。结果酒石酸布托啡诺喷剂喷鼻组的治疗有效率为97.62%，高于吗啡肌肉注射组的81.58%（P﹤0.05）；两组患者头晕、恶心和嗜睡等不良反应发生率比较，差异均无统计学意义（P﹥0.05）；治疗前，两组患者的睡眠质量比较，差异无统计学意义（P﹥0.05）；治疗后，两组患者的睡眠质量均较本组治疗前有改善（P﹤0.05），且酒石酸布托啡诺喷剂喷鼻组患者睡眠质量优于吗啡肌肉注射组（P﹤0.05）；酒石酸布托啡诺喷剂喷鼻组的治疗满意率为95.24%，高于吗啡肌肉注射组的78.95%（P﹤0.05）。结论酒石酸布托啡诺对晚期癌症患者自控镇痛有较好的效果，可明显改善患者的睡眠质量，具有临床应用价值。%Objective To investigate the effect of butorphanol tartrate in patient-controlled analgesia (PCA) in pa-tients with advanced cancer. Method 80 patients with advanced cancer pain were included in the study, and were ran-domized as intramuscular morphine group (n=38) and butorphanol nasal spray group (n=42), the therapeutic effects of the two groups were observed, and the incidence of adverse reactions, sleep quality and satisfaction degree with treatment were compared. Result The response rate of butorphanol nasal spray group was 97.62%, higher than that of morphine group at 81.58%(P<0.05);Incidences of dizziness, drowsiness and nausea and other adverse reactions were of no signifi-cant difference between the two groups (P>0.05);The sleep quality of patients in the two groups were similar before treat-ment (P>0.05), and were significantly improved after treatment (P<0.05), with better outcome observed in butorphanol nasal spray group as compared with intramuscular morphine group (P<0.05);For satisfaction degree with respective treat-ment, it was 95.24%in butorphanol nasal spray group, and 78.95%in intramuscular morphine group, with statistically sig-nificant difference observed (P<0.05). Conclusion Butorphanol tartrate is effective for patient-controlled analgesia in ad-vanced cancer patients, which significantly improves the quality of sleep, making it a clinically practicable therapy.
