心外科ICU应用不同吸痰装置的效果比较

摘要

目的:观察应用不同吸痰装置对心外科术后感染预防和控制效果的影响.方法:选择心外科术后行机械通气患者159例,随机分为密闭式吸痰组77例和开放式吸痰组82例.对比两组患者术后当天及术后第1,2,3 d的白细胞计数,ICU期间痰培养阳性率及带管时间.结果:密闭式吸痰组与开放式吸痰组在术后当天和第1 d的白细胞计数无显著差别(P>0.05),而密闭式吸痰组术后第2,3 d白细胞计数高于开放式吸痰组,差别有统计学意义(P<0.05).密闭式吸痰组痰标本细菌培养阳性率高于开放式吸痰组,带管时间长于开放式吸痰组,差别有统计学意义(P<0.05).结论:应用开放式吸痰装置发生感染率较低,更适合心外科术后短期带管患者.%Objective:To observe drainage effect of closed endotracheal suction and open endotracheal suction devices applied in cardiac surgery ICU. Methods: 159 patients after cardiac surgery with mechanical ventilation were randomly divided into the closed endotracheal suction group ( n =11 ) and the open endotracheal suction group ( n = 82 ). Compared the white blood cell count of two groups on the day and the first, second and the third days after surgery, and observed the positive rate of sputum culture before extubation and total duration of mechanical ventilation. Results: The white blood cell count was no significant difference between two groups on the day and the first day after surgery ( P >0.05 ),but it was significantly higher on the second and third days in closed endotracheal suction group( P <0.05 ). The positive rate of sputum culture in closed suction group was higher, and the total mechanical ventilation time of the closed suction group was longer than the open suction group,the difference was significant ( P <0.05 ). Conclusion:Open endotracheal suction has a more completely drainage effect.
