入院评估标示系统在降低老年住院患者护理不良事件发生率中的作用

摘要

Objective To discuss the effect of admission assessment indication system in reducing the inci-dence of adverse events in hospitalized older patients.Methods The fall (falling bed)risk assessment,risk assessment for pressure ulcer,intravenous extravasation risk factors assessment and catheter slippage risk assessment for patients with catheter detaining before or after admission were added to original admission assessment from July 2012,and take corresponding measures by hanging warning signs to the bed’s head and formulating the admission assessment indication system.The incidence of adverse events in hospitalized elderly patients were compared between before and after the application of system.Results After the appli-cation of admission assessment indication system,except pressure ulcer,the incidence of fall (falling bed), intravenous extravasation,and catheter slippage were all reduced compared with before the application of the system (all P<0.05).Conclusion Application of admission assessment indication system in hospitalized eld-erly patients can effectively reduce the incidence of adverse events and improve nursing safety.%目的：探讨入院评估标示系统在降低老年住院患者相关不良事件发生率中的作用.方法2012年7月解放军总医院南楼临床部开始在原有的入院评估中新增跌倒(坠床)危险因素评估、压疮危险因素评估、静脉液体外渗危险因素评估，对入院前或入院后留置管道者增加导管滑脱风险评估，并采取相应措施，在患者床头悬挂警示牌，形成入院评估标示系统，比较采用此系统前后老年住院患者相关不良事件的发生率.结果采用入院评估标示系统后，除压疮发生率外，患者跌倒(坠床)、静脉液体外渗、导管滑脱发生率均较实施前减少，差异有统计学意义(均P＜0．05).结论入院评估标示系统在老年住院患者中的应用，能有效降低相关不良事件的发生率，提高老年住院患者护理安全.
