托吡酯、卡马西平与丙戊酸钠治疗脑炎继发癫痫效果分析

摘要

Objective To analyze the effect on the encephalitis secondary epilepsy patients treated by topiramate (TPM) ,carbamaz‐epine (CBZ) and valproate (VPA) .Methods 96 cases encephalitis secondary epilepsy patients treated in our hospital from Febru‐ary 2012 to February 2014 were chosen as the research subjects .They were divided into TPM group(32 cases) ,CBZ group(32 ca‐ses) and VPA group(32 cases) by the random number table .The TPM group patients were treated by topiramate ,the CBZ group patients were treated by carbamazepine ,and the VPA group patients were treated by valproate .After 1 year ,the clinical efficacy and adverse reactions of the three groups patients were compared .Results The total effective rate of TPM group ,CBZ group and VPA group after treatment were 84 .38% ,78 .13% ,and 81 .25% ,respectively .There was no statistically significant difference . The adverse reaction incidence of the TPM ,the CBZ and the VPA group was 9 .38% ,37 .50% ,and 25 .00% ,respectively .There was statistically significant difference .Conclusions The effect on the encephalitis secondary epilepsy patients treated by TPM , CBZ and VPA is sure ,there is no difference in the curative effect among TPM ,CBZ and VPA .The encephalitis secondary epilep‐sy patients treated by topiramate showed significantly lower adverse reactions incidence than that by carbamazepine and valproate ,so topiramate is more suitable for use in clinic .%目的：探讨托吡酯（TPM）、卡马西平（CBZ）与丙戊酸钠（VPA）治疗脑炎继发癫痫（SEVE）的效果。方法选取我院2012年2月～2014年2月的96例脑炎继发癫痫患者为研究对象，采用随机数字表法将其均分为 T PM 组、CBZ组和V PA组。T PM组采用托吡酯，CBZ组采用卡马西平，V PA组采用丙戊酸钠进行治疗，疗程均为1年，观察3组患者治疗后的临床疗效以及不良反应情况。结果治疗后T PM组、CBZ组和V PA组的总有效率分别为84．38％，78．13％，81．25％，3组差异均无统计学意义；而T PM 组、CBZ组和V PA组的不良反应发生率分别为9．38％，37．50％，25．00％，差异具有统计学意义。结论托吡酯、卡马西平与丙戊酸钠治疗脑炎继发癫痫患者的临床效果确切，疗效间无差异，但是使用托吡酯的不良反应发生率明显低于卡马西平和丙戊酸钠。
