目的 观察立体定向血肿及脑室双靶点穿刺软通道引流法治疗高血压脑出血破入脑室系统的疗效.方法 对40例适合微创治疗的脑出血伴全脑室铸型患者行立体定向下血肿腔及血肿侧脑室双靶点穿刺,术后置软通道引流,必要时尿激酶纤溶冲管,神经外科常规治疗3 d复查脑CT,引流效果满意后分次拔除引流管,观察疗效.结果 经6个月随访,ADLⅠ级10例,ADLⅡ级14例,ADLⅢ级9例,ADLⅣ级4例,死亡2例,放弃治疗1例.结论 立体定向下血肿及脑室双靶点软通道外引流治疗脑出血破入脑室系统疗效好.%Objective It is to observe the curative effect of stereotactic induction hematoma and ventricular dual-target punctur and soft-channel drainage on hypertensive cerebral hemorrhage broken into the ventricular system. Methods Forty cases of cerebral hemorrhage patients with intraventricular hemorrhage cast who need minimally invasive treatment were treated by stereotactic intraventricular hematoma and the hematoma cavity dual-target lateral puncture, soft channel drainage after the operation, if necessary urokinase and plasminogen were used to douche tube, general neurosurgery treatment for 3 days and then the cranial CT was reexamed, drainage tubes were removed when drainage effect was satisfactory. Results After 6 months following-up, there were ADL Ⅰ 10 cases, ADL Ⅱ 14 cases, ADL Ⅲ 9 cases, ADLⅣ 4 cases, 2 dead cases, 1 abandoned treatment case. Conclusion Stereotactic induction hematoma and ventricular dual-target punctur and soft-channel drainage treatment for the cerebral hemorrhage broken into ventricles is an effective way.
