注射用灯盏花素对缺血性心肌病心室重构的影响

摘要

目的 观察注射用灯盏花素对缺血性心肌病患者心室重构(VR)的影响.方法 60例缺血性心肌病患者随机分为治疗组和对照组,每组30例.对照组仅进行常规治疗,治疗组在对照组基础上采用注射用灯盏花素.于治疗前后分别用超声心动图监测2组的左室射血分数(LVEF)、左室短轴缩短率(ΔFS)、每搏量(SV)、心排血量(CO)、心排血指数(CI)、二尖瓣快速充盈期和心房收缩期血流速度比值(E/A)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室舒张末期室间隔厚度(IVSTD)、左室后壁厚度(LVPWTD)、左室舒张末期容积(LVEDV)和左室收缩末期容积(LVESV);并观测治疗前后6min步行距离.结果 治疗组治疗后心功能、左室形态各项指标及6min步行距离较治疗前均有明显改善(P均<0.05),与对照组同期比较也均有显著性差异(P均<0.05);治疗组总有效率显著高于对照组(P<0.05).结论 注射用灯盏花素可在常规治疗基础上有效防治缺血性心肌病VR,延缓心力衰竭的发生和发展,较快改善缺血性心肌病患者心功能的恢复.%Objective It is to observe the influence of breviscapine for injection on ventricular remodeling ( VR ) in patient with ischemic cardiomyopathy.Methods Sixty patients with ischemic cardiomyopathy were chosen and randomly divided into treatment group and control group in which there were 30 cases.The control group was treated with routine treatment only.The treatment group was treated with hreviscapine for injection hased on the treatment of control group.The levels of left ventricular ejection fraction ( LVEF ), fractional shortening of left ventricular short axis ( △FS ),stroke volume ( SV ), cardiac output ( CO ), cardiac index ( CI ), the ratio of maximum blood flow between the advanced and early atrial systole ( E/A ), left ventricular end-diastolic dimension ( LVEDD ) , left ventricular end-systolic dimension ( LVESD ) , inlerventricular septum thickness diastolic ( lVSTD ) , left ventricular posterior wall thickness diastolic ( LVPWTD ) , left ventricular end diastolic volume ( LVEDV ) and left ventricular end systolic volume ( LVESV ) in the two groups before and after treatment were detected with ultrasonic cardiography; the 6-minute walk distance was observed before and after treatment.Results The heart function, all terms of left ventricular morphous indexes and 6-minute walk distance in treatment group after treatment were all improved compared with those before treatment ( all P ＜0.05 ).In addition, the improvement in treatment group was better than that in control group at the same time ( P ＜ 0.05 ).The total effective rate in treatment group was significantly higher than that in control group ( P ＜ 0.05 ).Conclusion Based on routine treatment, breviscapine for injection can effectively prevent and cure VR of ischemic cardiomyopathy, delay the occurrence and development of heart failure and relatively quickly improve the recovery of cardiac function in ischemic cardiomyopathy patients.
