Objective It is to explore the application value of CRP combined with ultrasonic measurement of fetal thymus transverse diameter in the diagnosis of preterm premature rupture of membranes with subclinical chorioamnionitis .Methods 45 patients with 28-36 +6 weeks pregnant singleton preterm premature rupture of membranes were selected and divided into pre -term premature rupture of membranes with chorioamnionitis group ( study groupⅠ) and preterm premature rupture of mem-branes without chorioamnionitis group ( study groupⅡ) according to placental pathological examination results .At the same 40 pregnant women who asked for termination of pregnancy because of other factors except premature rupture of membranes , pre-term labor without chorioamnionitis according to placental pathologic examination were selected as control group .The serum value of CRP and fetal thymus transverse diameter of the pregnant women in the three groups were measured .Results The level of CRP of study groupI was significantly higher while fetal thymus diameter measured values were lower than the other two groups (P <0.05).Serum CPR combined with fetal thymus diameter examination had high sensitivity and specificity in screening of preterm premature rupture of membranes with subclinical chorioamnionitis .Conclusion Serum CPR determination and fetal thymus diameter examination are helpful for the early diagnosis of preterm premature rupture of membranes with sub -clinical chorioamnionitis , the sensitivity and specificity were the highest when they were used together .%目的：探讨C反应蛋白（ CRP）及超声测量胎儿胸腺横径在未足月胎膜早破合并亚临床绒毛膜羊膜炎诊断中的应用价值。方法选取孕28～36＋6周单胎未足月胎膜早破孕妇45例，根据胎盘病理检查结果将45例孕妇分为未足月胎膜早破合并亚临床绒毛膜羊膜炎组和未足月胎膜早破不合并亚临床绒毛膜羊膜炎组。同时选取同期住院因除胎膜早破、早产以外的其他因素而终止妊娠，并且胎盘病理检查排除绒毛膜羊膜炎的孕妇40例为对照组。检测3组孕妇血清CRP值及经超声测量胎儿胸腺横径。结果未足月胎膜早破合并亚临床绒毛膜羊膜炎组的CRP水平明显高于其他2组（P均＜0．05），而胎儿胸腺横径明显低于其他2组（ P均＜0．05）。血清CRP与胎儿胸腺横径联合筛查（串联）未足月胎膜早破合并亚临床绒毛膜羊膜炎的敏感性及特异性较高。结论胎儿胸腺横径测量及孕妇血清CRP检测有助于早期诊断未足月胎膜早破合并亚临床绒毛膜羊膜炎，两者联合诊断（串联）亚临床绒毛膜羊膜炎的敏感性及特异性最高。
