Objective It is to observe the curative effect of Kangfuxin Liquid combined with bronchoscopic drug injection in the treatment of retreatment sputum smear positive cavitary pulmonary tuberculosis and its influence on immune function and respiratory function.Methods 126 cases of retreatment sputum smear positive cavitary pulmonary tuberculosis were randomly divided into observation group (63 cases) and control group (63 cases),on the basis of conventional treatment,the patients were treated with bronchoscopic drug injection in the control group,and with bronchoscopic drug injection combined with Kangfuxin Liquid in the observation group,6 months as one treatment course (2 months of reinforcement and 4 months of consolidation).The sputum negative conversion rate was observed in the two groups for 1-to 6-month,and the cavitary closure rate and the improvement of clinical symptoms at the end of the reinforcement period and consolidation period were recorded,and the immune function and pulmonary function index of the two groups were evaluated at the end of the reinforcement period and consolidation period.Results The sputum negative conversion rate in the observation group after treatment for 1-to 6-month was higher than those in the control group (all P ＜ 0.05);the cavitary closing rate at the end of the reinforcement period and consolidation period of the observation group were significantly higher than that of the control group (all P ＜ 0.05);the clinical symptoms (low fever,cough,expectoration,chest pain and loss of appetite) in the two groups were significantly improved at the end of the reinforcement period and consolidation period (all P ＜ 0.05),and the improvement of the observation group were significantly better than those of the control group (all P ＜ 0.05);the CD3 +,CD4 + and CD4,+/CD8 + at the end of the reinforcement period and consolidation period of the two groups were significantly higher while CD8 + were significantly lower than those before treatment (all P ＜ 0.05),and the improvement of the observation group was better than that of the control group (P ＜ 0.05);the lung function indexes of FVC,FEV1,FEV1/FVC and PEF were increased in the two groups at the end of the reinforcement period and consolidation period (all P ＜ 0.05),and these indexes in the observation group were higher than the control group (P ＜ 0.05).Conclusion Bronchoscopic drug injection combined with Kangfuxin Liquid in the treatment of retreatment sputum smear positive cavitary pulmonary tuberculosis can promote sputum negative conversion,accelerate cavity closure,and improve the clinical symptoms,immune function and pulmonary function,it issafe and reliable.%目的 观察康复新液联合支气管镜下注射药物治疗复治涂阳空洞型肺结核的疗效及对免疫功能和呼吸功能的影响.方法 将126例复治涂阳空洞型肺结核患者随机分为观察组63例及对照组63例,2组均给予常规治疗,对照组在常规治疗基础上给予支气管镜下注射药物治疗,观察组在对照组治疗基础上给予康复新液治疗,2组疗程均为6个月.观察2组治疗1～6个月的痰菌转阴率,记录2组治疗2个月末、6个月末的空洞闭合率以及临床症状改善情况,检测2组2个月末、6个月末的免疫功能、肺功能指标.结果 观察组在治疗1～6个月的痰菌转阴率显著高于对照组(P均＜0.05);观察组强化期末、巩固期末的空洞闭合率均显著高于对照组(P均＜0.05);2组2个月末、6个月末的临床症状(低热、咳嗽咳痰、胸痛、食欲不振)均显著改善(P均＜0.05),且观察组的改善情况优于对照组(P均＜0.05);2组2个月末、6个月末的CD3+、CD4+、CD4+/CD8+均较治疗前明显升高(P均＜0.05),CD8+较治疗前明显降低(P均＜0.05),观察组改善情况优于对照组(P均＜0.05);2组2个月末、6个月末的肺功能指标FVC、FEV1、FEV1/FVC、PEF均明显升高(P均＜0.05),观察组上述指标均明显高于对照组(P均＜0.05).结论 支气管镜下注射药物联合康复新液治疗复治涂阳空洞型肺结核能够促进患者痰菌转阴,加速空洞闭合,并显著改善患者临床症状、免疫功能及肺功能,安全可靠.
