清热活血方联合静脉溶栓治疗对冠心病急性心肌梗死患者炎性反应及心功能的影响

摘要

目的 探讨清热活血方联合静脉溶栓治疗对冠心病急性心肌梗死患者炎性反应及心功能的影响.方法 将98例冠心病急性心肌梗死患者随机分为2组,对照组48例予静脉溶栓治疗,研究组48例在对照组治疗基础上加用清热活血方,2组均持续治疗20 d.比较2组治疗前后总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、白细胞介素18(IL-18)、超敏C反应蛋白(hs-CRP)、心脏指数(CI)、心排血量(CO)、左心室射血分数(LVEF)变化情况,观察2组治疗后临床疗效.结果 治疗后,2组TC、TG、LDL-C水平均明显降低而HDL-C水平均明显升高(P均<0.05),且研究组血脂指标改善程度显著优于对照组(P均<0.05);2组IL-18、hs-CRP炎性因子水平均明显降低(P均<0.05),且研究组明显低于对照组(P均<0.05);2组LVEF、CI、CO心功能指标水平均明显升高(P均<0.05),且研究组明显高于对照组(P均<0.05);研究组总有效率显著高于对照组(P<0.05).结论 清热活血方联合静脉溶栓治疗冠心病急性心肌梗死患者,疗效显著,值得临床推广应用.%Objective It is to investigate the effect of Qingre-Huoxue decoction combined with intravenous thrombolytic therapy on inflammatory response and cardiac function in patients with acute myocardial infarction (AMI) of coronary heart disease (CHD).Methods 98 cases of patients with AMI of CHD were randomly divided into 2 groups, the control group (n=48) was treated with intravenous thrombolytic therapy, while the study group (n=48) was treated with Qingre-Huoxue decoction on the basis of treatment in control group.Both groups were treated for 20 days.The changes of levels of TC, TG, HDL-C, LDL-C, IL-18, hs-CRP, cardiac index (CI), cardiac output (CO), left ventricular ejection fraction (LVEF) before and after treatment were observed and clinical effects of the two groups after treatment were compared.Results After treatment, the levels of TC, TG and LDL-C were significantly decreased significantly (P<0.05), while that of HDL-C was increased in both groups (P<0.05), and the improvements of blood lipid indexes in the study group were better than that in the control group (P<0.05);the levels of IL-18 and hs-CRP were lower while cardiac function indexes such as LVEF, CI, CO were significantly higher in both groups compared with those before treatment(P<0.05), and the decrease or increase in the study group were more significant than those in the control group (P<0.05).The total effective rate of the study group was significantly higher than that in the control group (P<0.05).Conclusion Qingre-Huoxue decoction combined with intravenous thrombolytic therapy has good curative effect on AMI of CHD, it is worthy of clinical application.
