围术期异体输血对肺癌患者Th1/Th2、Th17/Treg细胞因子失衡的影响

Objective It is to observe the influence of perioperative allogeneic blood transfusion (ABT) on Th1/Th2,Th17/Treg cytokine imbalance in lung cancer patients.Methods A total of 64 patients with lung cancer were randomly divided into two groups,30 cases in the irradiated group were received irradiated suspension red blood cells and 34 cases in the nonirradiated groups were treated with ordinary suspension red blood cells.The immune status was assessed by flow cytometry (FCM) to detect the proportion of interferon gamma (IFN-γ,Th1 response),interleukin-4 (IL-4,Th2 response),interleukin-17 (IL-17,Th17 response) and forkhead box P3 (FOXP3,Treg response) in peripheral blood before and fourteenth days after operation.Results Fourteenth days after operation,the proportion of Th1,Treg cells and the ratios of Th1/Th2 were significantly increased (all P ＜ 0.05),and the proportion of Th2 cells and the ratios of Th17/Treg were significantly decreased (all P ＜ 0.05) in the two groups,the proportion of Th1 cells and the ratios of Th1/Th2,Th17/Terg of the irradiated group were significantly higher than those of the non-irradiated group (all P ＜ 0.05).Conclusion Thl/Th2,Th17/Treg cytokine imbalance after perioperative allogeneic blood transfusion maybe associated with active lymphocytes in suspension red blood cells.%目的 观察围术期异体输血后肺癌患者外周血CD4+T细胞各亚群Th1、Th2、Th17及Treg细胞相关因子的比例变化.方法 将行手术治疗的64例肺癌患者随机分为2组:未辐照组34例术中输入普通悬浮红细胞,辐照组30例术中输入辐照悬浮红细胞,分别于术前及术后14 d应用流式细胞术(FCM)测定2组患者外周血Th1(IFN-γ)、Th2(IL-4)、Th17(IL-17)及Treg(Foxp3)细胞比例.结果 术后14d,2组外周血中Th1、Treg细胞比例和Th1/Th2均明显高于术前(P均＜0.05),Th2细胞比例、Th17/Treg均明显低于术前(P均＜0.05),且辐照组Th1细胞比例及Th1/Th2、Th17/Treg比值均显著高于未辐照组(P均＜0.05),Treg细胞比例低于未辐照组(P＜0.05).结论 肺癌患者围术期异体输血后Th1/Th2、Th17/Treg细胞因子失衡可能与输入异体血中的活性淋巴细胞有关.
