帕金森患者髋骨骨折与髋部骨密度值的相关性分析

摘要

目的：探讨帕金森病（Parkinson Disease，PD）患者髋骨骨折与髋部骨密度的相关性。方法探讨帕金森病患者髋骨骨折与髋部骨密度的相关性。方法连州市人民医院2009年3月～2012年3月的门诊帕金森患者123例作为帕金森组（PD组），体质量指数平均为（22.04±3.78）kg／m2。根据Wester评分，将PD组患者进一步分为：轻度组（1～10分）、中度组（11～20分）、重度组（21～30分）。收集老年健康体检人员125例作为对照组（OB 组），体质量指数平均为（21.94± 3.69）kg／m2。检测两组患者髋部股骨颈、转子、转子间、整髋、Ward区5个区域的骨密度值及其平均值，并进行对比分析。对所有观察对象进行持续3年的跟踪随访，记录随访期间髋部骨折的发生情况，对比不同组骨折发病率的差别。结果 PD与OB两组患者在髋部5个区域的骨密度值及其平均值对比均有显著性差异，均 p＜0．05。 PD重度组髋部5个区域的骨密度值及其平均值均低于PD轻度组与中度组，均p＜0．05；PD轻度组与中度组比较有显著性差异，p＜0．05。 PD组髋部骨折发生率高于OB组，p＜0．05；PD重度组髋部骨折发生率高于轻度组、中度组，均p＜0．05；PD轻度组与中度组骨折发生率无显著性差异，p＞0．05。结论帕金森患者髋部骨密度值下降，并与高骨折率成正相关。帕金森病情越重髋部骨密度值下降越明显，骨折发生率越高。%Objective To investigate the correlation of the hip fracture in Parkinson disease patients with the hip bone miner-al density.Methods 123 cases of outpatient Parkinson disease as Parkinson group ( PD group) were collected from March 2009 to March 2012 at Lianzhou city people's hospital.The average body mass index (BMI) was (22.04 ±3.78)kg/m2.According to Mr Wester score, the PD group patients were further divided into: the mild group (1 ~10 scores), moderate group (11 ~20 scores) and severe group (21 ~30 scores).And 125 cases were collected from health examination department as control group(OB group), The average body mass index (BMI) was (21.94 ±3.69)kg/m2.We detected and analyzed the bone mineral density (BMD) of the two groups at hip femoral neck, trochanter, intertrochanteric, whole hip, Ward area and the average of the five regional above.All the subjects continued 3 years following-up and the accidents of hip fracture were recorded.Results The BMD at hip femoral neck, trochanter, intertrochanteric, whole hip, Ward area and the average of the five regional between the PD and OB group were signifi-cantly different, p<0.05.The BMD of PD severe group were lower than PD mild group and moderate group, p<0.05.And the BMD of PD moderate group were lower than PD mild group, p <0.05.The incidence of hip fracture of PD group were higher than OB group, p<0.05.The incidence of hip fracture of PD severe group were higher than PD mild group and moderate group, p<0.05. And the incidence of hip fracture between PD moderate group and PD mild group were no significantly different, p>0.05.Conclu-sion The hip BMD of Parkinson disease patients decreased, and the correlation is positive with high rate of fracture.The heavier Parkinson's disease is, the more obvious the hip BMD decreased, and the higher the incidence of fracture.
