衢州市某医院糖尿病肾病腹膜透析患者容量管理能力及社会功能研究

摘要

目的:研究糖尿病肾病腹膜透析患者容量管理能力及社会功能的现状,并探讨两者之间的相关性,为提高患者的容量管理能力和社会功能提供理论依据.方法:选择接受腹膜透析的糖尿病肾病患者192例作为研究对象.收集患者基本信息,采用腹膜透析患者容量管理量表评估患者的容量管理能力,采用社会功能缺陷筛选表评估患者的社会功能状况.结果:192名糖尿病肾病腹膜透析患者容量管理能力总分为(13.28±5.41)分,其中饮食管理维度得分为(3.31±1.52)分,相关指标及并发症监测维度得分为(9.97±4.89)分;患者性别、居住地、透析时间和残存尿量均是影响患者容量管理能力的影响因素(P＜0.05).社会功能平均得分(3.23±1.57)分,63.02％的患者存在社会功能缺陷;社会功能得分越高患者容量管理能力得分越低(r=-0.46,P＜0.01).结论:糖尿病肾病腹膜透析患者容量管理能力有待提高,一半以上患者存在社会功能缺陷.医护人员应注重患者的社会功能状况,提高患者容量管理能力.%Objective:To investigate the capacity management and social functions in peritoneal dialysis patients with diabetic nephropathy,and to explore the correlation between them,in order to provide clinical intervention basis for improving the capacity management ability.Methods:192 cases of diabetic nephropathy patients undergoing peritoneal dialysis were selected as the subjects who received treatment in a Hospital in Quzhou City.Self-designed questionnaire was used to collect the basic situation.The basic information of patients was collected.The volume-management Scale for peritoneal dialysis patients was applied to evaluate the patients' capacity management competence.The social functioning defect screening form was applied to evaluate the patients' social functions.Results:The score of volume-management scale with diabetic nephropathy was (13.28±5.41),the dietary management dimension was (3.31± 1.52),the correlation index and the monitoring dimension were (9.97±4.89);Patients gender,residence,dialysis time and residual urine volume are important factors affecting the capacity management (P＜0.05).The score of social functions of the patients with diabetic nephropathy was (3.23± 1.57),63.02％ of patients had social dysfunction.The higher the score of social function,the lower the score of capacity management (r=-0.46,P＜0.01).Conclusion:Capacity management of peritoneal dialysis patients with diabetic nephropathy needs to be improved.More than half of the patients had social dysfunction,and the two were negatively correlated.Medical staff should pay attention to the social function of patients,thereby improving the capacity management capacity of patients.
