斑点追踪成像技术对原发性高血压伴舒张性心力衰竭患者左心室整体收缩功能的临床评价

             

摘要

Objective To evaluate the value of two ̄dimension speckle tracking imaging in assessing left ventricular global systolic function in diastolic heart failure in patients with essential hypertension. Methods The study included 65 patients with essential hypertension and preserved left ventricular systolic function (LVEF >50%),which were divided into patients with diastolic heart failure (n=26,DHF group) and those without heart failure(n=39,NHF group).26 age matched healthy controls were selected randomly.Two ̄dimensional echocardiographic images of the left ventricle were acquired in apical four ̄,two ̄chamber,long ̄axis views,and parasternal short ̄axis at the basal,mid,and apical levels.Global longitudinal strain (GLS), global radial strain(GRS), global circumferential strain(GCS) were quantified using a novel speckle tracking sys ̄tem.Global parameters were investigated among the groups. Results Compared with the control group,the value of GLS, GRS,GCS were significantly reduced in DHF group and NHF group( P<0 05);Compared with the NHF group,the value of GLS,GRS,GCS were significantly reduced in DHF group ( P<0 05) . Conclusions 2D ̄STI can evaluate 1eft ventricular global systolic function in diastolic heart failure in patients with essential hypertension,and has potential applications in clini ̄cal practice.%目的:探讨二维斑点追踪成像技术(two ̄dimensional speckle tracking imaging,2D ̄STI)对原发性高血压伴舒张性心力衰竭患者左心室整体收缩功能的临床评价。方法将本院65例原发性高血压患者(左室射血分数( LVEF)>50%),分为舒张性心力衰竭组( DHF,n=26)和非舒张性心力衰竭组( NHF,n=39)。同时选择年龄相当的26名健康人为对照组。常规二维超声心动图获取心尖四腔、二腔观、左心室长轴及胸骨旁二尖瓣水平、乳头肌水平及心尖水平左心室短轴观的高帧频动态灰阶图像,得到左心室整体纵向应变(GLS)、圆周应变(GCS)、径向应变(GRS)。比较三组间上述指标的差异。结果与对照组比较,DHF组及NHF组的GLS、GCS、GRS绝对值均明显减低( P<0.05)。与NHF组比较,DHF组的GLS、GCS、GRS绝对值均明显减低( P<0.05)。结论2D ̄STI技术可应用于对原发性高血压伴舒张性心力衰竭患者左心室整体收缩功能进行评估,为临床提供有价值的信息。
