脐血流检测联合胎心监测对妊娠期高血压疾病患者围生结局的预测研究

摘要

目的：探讨脐血流检测联合胎心监测对妊娠期高血压疾病患者围生结局的预测结果。方法选取2011年6月至2013年6月西安市第九医院和西安市天伦不孕不育医院收治的80例住院分娩的妊娠期高血压疾病孕妇作为观察组，同期选择80例无妊娠高血压疾病等并发症的孕妇作为对照组。对两组孕妇进行脐血流检测和胎心监测，记录并比较围生结局，分析脐血流检测联合胎心监测对妊娠期高血压疾病患者预后的影响。结果观察组胎儿的胎儿脐动脉收缩压与舒张压的比值（ S／D）≥3和胎心基线异常比例显著高于对照组，胎儿预后不良比例显著高于对照组，差异有统计学意义（ P＜0.05）。 S／D≥3的胎儿出生后正常率显著低于S／D＜3的胎儿，差异有统计学意义（ P＜0.05）；无刺激胎心监测（NST）反应型胎儿出生后的正常率显著高于 NST无反应型、可疑型，差异有统计学意义（ P＜0.05）。结论对妊娠期高血压疾病孕产妇采用脐血流检测联合胎心监测，能够及时了解胎儿宫内情况，具有无创性，可准确对围生结局进行预测。%Objective To investigate the prediction of perinatal outcome by pregnancy flow detection combined with fetal monitoring in patients with gestational hypertension .Methods A total of 80 cases of hospitalized gestational hypertension in Xi′an Ninth Hospital and Xi′an Tianlun Infertility Hospital during Jun.2011 and Jun.2013 were selected as observation group,and 80 cases of hospitalized delivery without hypertension during the same period were included as control group.The influence of results of umbilical blood flow detection combined with fetal heart monitoring on perinatal outcome , prognosis of hypertensive patients were recorded and compared.Results In the observation group,fetal systolic blood pressure/dias-tolic blood pressure( S/D) score ≥3 and fetal heart rate baseline abnormalities were significantly more than those in the control group,the adverse prognosis for the fetus were significantly more than those in the control group,with statistically significant difference( P<0.05 ) .The postpartum normal rate of fetus with S/D≥3 was significantly lower than the fetus with S/D <3 ( P <0.05 ); the postpartum normal rate of fetus with NST-responsiveness was significantly higher than the fetus with no NST-responsiveness or uncertain NST-responsiveness( P<0.05 ) .Conclusion Umbilical blood flow detection combined with fetal heart monito-ring for gestational pregnancy,is non-invasive,accurate to predict the perinatal outcome.
