目的 探讨128层螺旋CT三维重建应用于婴幼儿脑外伤的优势.方法 对152例脑外伤患儿行128层螺旋CT扫描,对照5mm层厚的图像与薄层三维重建后的诊断结果.结果 螺旋CT薄层三维重建对颅骨骨折检出率从69%提高至100%,并且对骨膜下血肿、副鼻窦和乳突的积血、脑实质内出血、头皮下血肿显示率有不同程度提高.结论 128层螺旋CT三维重建不仅精确地显示骨折结构的空间关系,而且明显提高骨折伴发病灶的检出,较传统CT缩短检查时间及减少辐射剂量,临床应用具备明显优势.%Objective To study the advantage of 128-slice spiral CT and 3D-Reconstruction used in brain injury of infant.Methods 152 cases of children with brain injury were scanned by 128 multi-section spiral CT,the imaging (thick 5 mm) results were compared to those of thin layer 3D-reconstruction.Results The detection rate of spiral CT in skull fracture (100%) was higher than that of 3D-reconstruction (69%),so did the detection rate in subperiosteal hematoma,paranasal sinus and papillate hemorrhage,and parenchymal hemorrhage.Conclusion 128-slice spiral CT and 3D-reconstruction is not only accurately display the fracture structure of the space relation,but also obviously increase fracture concomitant focal detection,shorten the testing time and reduce the radiation dose compare to the traditional CT.It has obviously advantage of clinical application.
