原发性肾上腺恶性淋巴瘤的临床病理学观察

摘要

目的讨论原发性肾上腺恶性淋巴瘤的临床及病理形态学特点、诊断及鉴别诊断。方法对2例原发性肾上腺恶性淋巴瘤病进行临床、病理组织学及免疫组织化学观察并结合文献复习。结果本瘤无特异性，细胞学上与他处的结内、外淋巴瘤的细胞学特点相似，组织学上为弥漫性或片状分布。免疫表型LCA(+)、B细胞或T细胞抗原（+），上皮和神经内分泌表型（-）。结论原发性肾上腺恶性淋巴瘤是一种较罕见的恶性肿瘤，早期症状无特异性，发现晚，预后差，主要靠组织学和免疫组织化学确诊。应注意与继发性恶性淋巴瘤及其他上皮或神经内分泌肿瘤鉴别。%Objective To describe the clinicopathologcal feature and diagnosis of primary adrenal lymphoma. Methods Clinical data,pathological change and immunochemical finding were obseved,and dif erential diagnosis was disecused in the two cases of primary adrenal lymphoma by reviewing of the laterature. Results The preseting symptoms were nonspective.Histological y,the tumor cel s showed a round or oval shape,formed the structure of solid pat ern or diffuse sheet,and same withe other lymphoma in nodular or non.Immunohistochemical y,the tumor cel were positive for CD45,B-cel or T- cel antigen,but negtive for CK, CEA, cgA, syn and s-100 protien.Conclusion The primary adrenal lymphoma is anextramel y rare tumor with a high grade of malignancy and poor prognosis.the early symptoms is nonspecific.Its diagnosis and dif erential diagnsis depend on histolgy and immunohistochemestry.It should be dif erntiated from the secondry lymphoma and other epithelial or neural endocrine neoplasmas.
