9项呼吸道病原体联合检测在儿科呼吸道疾病诊断中的应用

         

摘要

Objective To investigate the application value of IgM antibodies detection of 9 respiratory tract pathogens in the di‐agnosis of child acute respiratory tract diseases .Methods Totally 672 pediatric cases in our hospital from January to November 2015 were selected as the study group and contemporaneous 100 healthy children were selected as the control group .The venous blood at the initial onset stage in the children patients with respiratory tract infection and control group were collected .The IgM an‐tibodies of Legionella pneumophila(LP) ,mycoplasma pneumoniae(MP) ,Coxiella burnetii (COX) ,chlamydia pneumoniae(CP) ,ade‐novirus(ADV) ,influenza virus type A(IFA) ,influenza virus type B(IFB) ,parainfluenza virus(PIV) and respiratory synthetic syn‐cytial virus (RSV) .The occurrence rates of 9‐item pathogen infection in different seasons of winter ,spring ,summer and autumn were investigated and the difference of their infection among different age groups was analyzed .Results Among 672 children cases , the positive detection rate of pathogens was 42 .6% (286/672) ,the detection rate of single pathogen infection was 36 .3% (244/672) ,the detection rate of mixed pathogen infection was 9 .4% (63/672) ,the difference was statistically significant compared with the control group(P<0 .05) .The IgM antibody positive detection rates of 9 respiratory tract pathogens in spring ,summer ,autumn and winter were 57 .5% ,20 .4% ,42 .3% and 44 .4% respectively ,which in spring was the highest and which in summer was the lowest .The IgM antibody positive detection rates of 9 respiratory tract pathogens in the age groups of 0 - <1 year old ,1 - <3 years old ,3-5 years old and >5 years old were 55 .3% ,37 .5% ,31 .7% and 30 .3% respectively ;which in the 0 - <1 year old group was the highest and which in the >5 years old group was the lowest .The MP and PIV infection were common in different age groups and different seasons .Conclusion MP and PIV are common atypical pathogen infection in pediatrics ,winter and spring are the high onset seasons of respiratory tract pathogen infection ,infants aged less than 1 year old are the main onset objects . Strengthening the 9‐item respiratory tract pathogen infection monitoring has great application value and is conducive to the diagno‐sis and treatment of child acute respiratory tract diseases .%目的:研究9项呼吸道病原体Ig M抗体检测在儿科急性呼吸道疾病诊断中的应用价值。方法选择2015年1-11月收治的儿科病例672例为研究组,同时选择100例同期健康儿童为对照组,采集呼吸道感染患儿发病初期静脉血及对照组儿童静脉血血清,采用免疫荧光法进行嗜肺军团菌(LP),肺炎支原体(MP),Q热立克次体(COX),肺炎衣原体(CP),腺病毒(ADV),甲型流感病毒(IFA),乙型流感病毒(IFB),副流感病毒(PIV)1型、2型和3型,呼吸道合胞病毒(RSV)9项病原体IgM 抗体阳性检查,探讨春夏秋冬不同季节9项病原体感染的发生率,并分析不同年龄段的儿科病例9项病原体感染的差异性。结果研究组672例患儿病原体感染检测阳性率为42.6%(286/672),单项病原体感染检出率为36.3%(244/672),混合病原体感染检出率为9.4%(63/672),与对照组比较差异有统计学意义( P<0.05)。春季、夏季、秋季、冬季呼吸道9联病原体Ig M 抗体检测阳性率分别为57.5%、20.4%、42.3%、44.4%,其中春季检测阳性率最高,夏季检测阳性率最低。0~<1、1~<3、3~5、>5岁年龄段呼吸道9联病原体Ig M抗体检测阳性率分别为55.3%、37.5%、31.7%、30.3%;其中0~<1岁年龄段检测阳性率最高,>5岁年龄段检测阳性率最低。在不同季节及不同年龄阶段中M P与PIV病原体感染均比较常见。结论 M P与PIV是儿科常见的呼吸道非典型病原体感染,冬季与春季是呼吸道病原体感染的高发季节,年龄小于1岁的婴幼儿是主要的发病对象。加强9项呼吸道病原体感染检测具有比较大的应用价值,有益于儿科急性呼吸道疾病的诊断及治疗。
