阿托伐他汀联合美托洛尔应用于慢性心功能衰竭患者对其T细胞功能及临床预后的研究

摘要

Objective To study The impact of T cell function and clinical outcomes with atorvastatin combined with metoprolol in patients with chronic heart failure.Methods 92 patients with chronic heart failure from January 2016 to March 2016 in Cardiology department of our hospital were distributed into observation group and control group.Observation group were given atorvastatin combined with metoprolol, the control group only received metoprolol.Results Before treatment, the two groups were the same in both Th1, Th2,CD4+cell and CD8+cell levels, LVEF and NT-proBNP is also in the similar level(P>0.05).After treatment, Th1 and CD8+cell decreased, Th2 and CD4+ cell were significantly higher than the level of before treatment and control group after treatment (P<0.05).Accordingly, the levels of LVEF and NT-proBNP also declined with different degree, but the extent of the observation group decreased relatively more (P<0.05).In terms of clinical outcomes of the two groups: in the observation group, we found that overall effective up to 91.78% while in the control group was only 74.66% (P<0.05).Conclusion Atorvastatin combined with metoprolol in patients with CHF can be applied to regulate Th1 and Th2 imbalance, reverse the cardiac remodeling, there by improve the symptoms of heart failure, ejection fraction and clinical outcomes.%目的 分析阿托伐他汀联合美托洛尔应用于慢性心功能衰竭(chronic heart failure,CHF)患者对其T细胞功能及临床预后的研究.方法 选择2016年1月至2016年3月在我院心内科收治的慢性心功能衰竭的患者292例,按照随机、双盲的原则,分为观察组与对照组.观察组给予阿托伐他汀片联合美托洛尔,对照组仅给予美托洛尔.结果 治疗前,两组患者Th1、Th2、CD4+T细胞、CD8+T细胞均在同一水平,二者比较无明显差异(P>0.05).治疗后,观察组与治疗前相比Th1、CD8+T细胞均明显降低(P<0.05)、Th2、CD4+T细胞均显著升高(P<0.05);且和治疗后的对照组比较,差异具有统计学意义(P<0.05);治疗后,两组患者的LVEF及NT-proBNP也均有不同程度的下降,但观察组下降程度相对更为明显,差异具有统计学意义(P<0.05).观察两组的临床结局,发现观察组总体有效达91.78%,对照组却仅为74.66%,二者比较具有统计学差异(P<0.05).结论 阿托伐他汀联合美托洛尔应用于CHF患者可调节T细胞比例失衡,逆转心肌重构,从而改善心衰症状,提高射血分数,改善临床转归.
