降钙素调节BMP-2治疗骨质疏松所致骨折的临床价值分析

摘要

Objective To investigate the effect of BMP-2 on calcitonin in the prevention of osteoporosis caused by chondrocyte apoptosis.Methods Sixty patients with femoral intertrochanteric fractures or femoral neck fractures caused by osteoporosis in our hospital from May 2014 to May 2017 were selected and divided into control group and observation group.The control group were given normal operation and internal fixation,and the observation group were given salmon calcitonin nasal spray which was a daily spray to the fracture healing based on normal operation and internal fixation.The fracture healing was assessed by Flynn joint function evaluation criteria.The expression levels of BMP-2,NF-κB and BCL-2 in serum were detected by enzyme-linked immunosorbent assay(Elisa).The expression of Caspase3 was detected by Caspase3 activity assay kit. The expression of BMP-2 in bone and joint was detected by immunohistochemistry.Results The score of callus growth in the observation group was significantly higher,and the excellent and good rate of Flynn function evaluation was significantly increased at 3 months after operation(P<0.05).The levels of serum BMP-2 and BCL-2 in the observation group were significantly increased,and NF-κB and Caspase3 were significantly decreased(P <0.05).Conclusion BMP-2 plays an important role in the treatment of osteoporotic fractures.Salmon calcitonin nasal spray given to patients can effectively assist in the treatment of osteoporosis-induced fractures to help promote fracture healing.%目的 探讨BMP-2在降钙素抑制软骨细胞凋亡治疗骨质疏松所致骨折的作用.方法 选取本院2014年5月至2017年5月收治的骨质疏松导致的股骨转子间骨折或股骨颈骨折患者60例,分为对照组和观察组.对照组给予正常手术内固定治疗,观察组在此基础上给予鲑鱼降钙素鼻喷剂,每日一喷至骨折愈合.以Flynn关节功能评价标准评定患者骨折愈合情况;采用酶联免疫吸附法(ELISA)检测患者血清中BMP-2、NF-κB和BCL-2表达;采用Caspase3活性检测试剂盒检测患者血清中Caspase3表达;采用免疫组织化学法检测患者骨关节BMP-2表达.结果 与对照组相比,观察组患者的骨痂生长情况评分均明显提高,术后3个月Flynn 功能评价优良率明显提高,差异具有统计学意义(P<0.05);观察组患者血清BMP-2和BCL-2水平显著上升,NF-κB和Caspase3明显降低,差异具有统计学意义(P<0.05).结论 BMP-2在骨质疏松所致骨折的治疗过程中起到重要的作用;给予患者鲑鱼降钙素鼻喷剂可有效辅助治疗骨质疏松诱导的骨折,帮助促进骨折愈合.
