严重创伤救治的损害控制外科策略

摘要

严重创伤后，常出现严重的生理功能紊乱和机体代谢功能障碍，导致低体温、凝血病、代谢性酸中毒等“致命三联征”，机体因生理功能耗竭而不能承受长时间的确定性手术。在严重创伤患者抢救中，适时实施损害控制外科策略，通过采取各种快速、有效的暂时性措施，维持患者基本的生命功能，再进入ICU进行复苏和纠正生理功能紊乱，有计划地分次进行确定性治疗，对降低死亡率、提高救治成功率具有重要的临床实用价值。%Severe trauma patients often occur severe physiological disorders and metabolism dysfunction , leading to the “deadly triad” of hypothermia,coagulopathy and metabolic acidosis .Patients can’t withstand long-time surgery due to the poor physiological functions of the body .The strategy of damage control surgery is to main-tain the patients ’ basic life functions through a variety of fast and effective temporary measures ,then conduct resus-citation and correct physiological dysfunction in ICU and finally to perform the definitive treatment .During rescuing the severe trauma patients , applying the strategy of damage control surgery can reduce mortality and improve the treatment success rate .
