解剖M型超声心动图在冠心病诊断中的应用

         

摘要

目的 探讨解剖M型超声心动图在冠心病诊断中的临床应用价值.方法 对599例临床拟诊冠心病的患者行解剖M型超声心动图检查,观察室壁有无节段性运动异常,并对上述病例行冠脉造影检查,比较不同狭窄程度及不同狭窄部位情况下两者诊断的一致性.结果 冠状动脉狭窄Ⅲ级情况下,解剖M型超声心动图显示室壁节段性运动幅度减低对冠心病的诊断与冠脉造影相比二者具有较好的一致性(K=0.804,P<0.001),不同部位冠脉狭窄解剖M型超声心动图表现与冠状动脉造影检测有很高的一致性(K=0.761,P<0.001).结论 解剖M型超声心动图在冠心病诊断和冠脉造影术前筛查方面具有重要的临床意义,是一项首选的无创性检查方法.%Objective To investigate the value of anatomical M-mode echocardiography in the diagnosis of coronary heart disease. Methods Anatomical M-mode echocardiography was taken in 599 cases of suspected coronary heart disease to observe the segmental motion abnormalities in the ventricular wall. Meanwhile, the consistency between coronary angiography and echocardiography in the diagnosis of different degrees and different parts of stenosis was compared. Results Anatomical M-mode echocardiography had a good consistency with coronary angiography in the diagnosis of Grade Ⅲ coronary stenosis ( K = 0. 804,P <0. 001 ). Also,anatomical M-mode echocardiography had a high consistency with coronary angiography in the diagnosis of stenosis in different parts of coronary artery( K = 0.761 ,P <0.001 ). Conclusion Anatomical M-mode echocardiography is a preferred noninvasive method in the diagnosis of coronary heart disease and coronary angiography preoperative screening.
