阿片类药物在剖宫产麻醉镇痛中的应用研究

         

摘要

Objective To investigate the clinical narcotic effect of sufentanil for patients in cesarean section.Methods All 150 patients who received cesarean section were randomly divided into control group (n =75) and treatment group (n =75).The treatment group was given bupivacaine combined with sufentanil anesthesia,and the control group was given bupivacaine anesthesia.The sensory block and motor nerve block,the anesthetic effect,the Apgar score,and adverse reactions of 2 groups were observed and compared.Results The onset time of sensory block in treatment group was significantly shorter,the duration was significantly longer,and the highest plane of sensory block was significantly higher than that in control group (P < 0.05) ;the maximum time of motor block in treatment group was significantly longer,the degree of motor block score was significantly lower,and the recovery time was significantly shorter than that in control group (P < 0.05) ; the satisfaction rate of narcotic analgesic in treatment group was significantly higher than that in control group (P < 0.05) ; there was no significant difference of Apgar score in 2 groups (P > 0.05) ; the incidence of hypotension,bradycardia,traction reaction,chills in the treatment group were significantly lower than those in control group (P < 0.05).Conclusion The sufentanil in cesarean section can improve the anesthetic effect and reduce adverse reactions.%目的 探讨阿片类麻醉镇痛药舒芬太尼用于剖宫产麻醉镇痛的临床效果.方法 将纳入标准的150例择期行剖宫产的患者随机分成实验组和对照组各75例,实验组采用布比卡因复合舒芬太尼腰硬联合阻滞麻醉,对照组单纯采用布比卡因腰硬联合阻滞麻醉.观察比较2组的感觉和运动神经阻滞情况、麻醉镇痛效果、新生儿Apgar评分及不良反应发生情况.结果 实验组的感觉阻滞起效时间明显短于对照组,感觉阻滞持续时间明显长于对照组,感觉阻滞最高平面明显高于对照组(P<0.05);实验组达最大运动阻滞时间明显长于对照组,运动阻滞程度评分明显低于对照组,运动阻滞恢复时间明显短于对照组(P<0.05);实验组的麻醉镇痛满意率明显高于对照组(P<0.05);2组的新生儿Apgar评分无显著性差异(P>0.05);实验组的低血压、心动过缓、牵拉反应、寒战等发生率均明显低于对照组(P<0.05).结论 阿片类药物舒芬太尼用于剖宫产能改善麻醉镇痛效果,减少不良反应发生.
