IgA肾病中肾小动脉增厚及其临床评价

摘要

Ojective Analyses the changes of the glomearular arterioles and the correlation between arteriolar thickening with clinical or pathological features in patients with IgA nephropathy.Methods 110 patients with biopsy proven IgA nephropathy were I-dentified from Renal Disease database.Vascular measurement applications of medical image analysis system single-blind method for cal-culating the cavity diameter and blood vessels.Results 1.The characteristics of arteriolar thickening was vessel smooth muscle hypertrophy and/or hyaline deposit in arteriolar wall, both pathological changes can cause lumenal narrowing.2.There was a strong correlation (T2 =2.851 ,P<0.05) between the arteriolar thickening index and tubulo interstitial lesions.Conclusion The arteriolar thickening index was strongly correlated with tubulointerstitial lesions.It reflects the disease degree and can be used as an important histolog-ical prognostic indicator.It might be involved in the pathogenesis of renal fibrosis and the development of chronic renal failure.%目的:目的观测IgA肾病肾小动脉病理学改变,探讨小动脉增厚与临床症状及病理学改变的关系.方法:选内蒙古地区汉族110例IgA肾病患者,医学图像分析系统单盲法测算小动脉血管腔内径与血管外径比值,分析小动脉增厚与临床症状及病理学改变的关系.结果:①IgA肾病小动脉增厚可表现为肌层增厚或玻璃样变性,造成管腔狭窄,两者可共存.②IgA肾病小动脉增厚血压正常者小动脉增厚系数与小管间质损伤指数密切相关(P<0.05).结论:IgA肾病小动脉增厚与小管间质损伤密切相关,能反映病变的程度,可作为判断预后及干预治疗的一个重要病理指标.
