Mippo技术置入锁定加压钢板治疗 Pilon 骨折疗效观察

摘要

Objective To observe the clinical efficacy of minimally invasive percutaneous plate fixation (MIPPO) combined with locking compression plate (LCP) in the treatment of Pilon fracture.Methods 120 cases admitted in our hospital during January 2012 and January 2015 because of Pilon fractures were enrolled in this study.They were divided into two groups according to the surgical method .In control group, 60 patients taking the traditional incision fixation method for treatment , while in observation group , 60 patients were treated with minimally invasive percutaneous plate fixation ( MIPPO) combined with locking compression plate ( LCP ) treatment.The average length of incision, blood loss, mean operation time, average length of stay, the average healing time of the two groups were compared .All patients were followed up for 1 year, the occurrence of breakage of internal fixation , nonunion , screw cut out , infection , deep vein formation and other complications were compared.Results Compared with the control group , patients in observation group with small incision , less bleeding and shorter operative time , there are statistically significant .Excellent and good rate of the observation group ( 75.0%) was significantly higher than 45.0% of the control group , the difference was statistically significant ( P<0.05 ) .The difference of postoperative complications between the two groups was statistically significant.Conclusions The application effect of MIPPO combined LCP in the treatment of Pilon fracture is significant , it can shorten the operation time , promote the early resumption of fracture , significantly reduce postoperative incidence of adverse reactions .%目的：观察微创经皮钢板内固定术（MIPPO）结合锁定加压钢板（LCP）治疗Pilon骨折的临床疗效。方法选取本院2012年1月至2015年1月收治入院的Pilon骨折患者120例作为研究对象，根据手术方式不同分为两组，对照组60例患者采取传统切开复位内固定方法治疗，观察组60例患者给予经皮钢板内固定手术（MIPPO）结合锁定加压钢板（LCP）治疗，比较两组患者切口平均长度、术中出血量、平均手术时间、平均住院时间、平均骨折愈合时间等情况，所有患者均随访1年，比较两组内固定物断裂、骨不连、螺钉切出、感染、形成下肢深静脉血栓等并发症发生情况。结果观察组较对照组切口小，出血量少，手术时间短，有统计学意义；观察组患者治疗的优良率为75．0％．明显高于对照组的45．0％，差异明显，有统计学意义（ P＜0．05）；两组术后并发症比较有统计学意义。结论 MIPPO结合LCP 治疗Pilon骨折疗效显著，可缩短手术时间，促进骨折尽快恢复，明显降低术后不良反应发生率。
