血液灌流联合血液透析在百草枯中毒中的运用及疗效

摘要

目的：探讨血液灌流联合血液透析在百草枯中毒中的运用及疗效。方法将来自2012年1月至2015年12月的64例百草枯中毒患者分为实验组（32例）和对照组（32例），实验组采用血液灌流（HP）联合血液透析（HD），而对照组采用HP治疗，统计两组的疗效、血药浓度、尿素氮（BUN）、肌酐（Cr）、丙氨酸氨基转移酶（ALT）、肿瘤坏死因子－α（TNF－α）、转化生长因子－β1（TGF－β1）、血小板衍生因子（PDGF）、基质金属蛋白酶－9（MMP－9）和血氧分压（PaO2），采用卡方检验和t检验分析相关数据。结果实验组总有效率明显高于对照组（P＜0．05）；治疗后，两组的血药浓度、BUN、Cr、ALT、TNF－α、TGF－β1、PDGF、MMP－9均明显下降，但实验组的血药浓度、BUN、Cr、ALT、PDGF、MMP－9和PaO2下降的幅度明显大于对照组（P＜0．05）。结论应用HP联合HD治疗百草枯中毒的疗效明确，同时可以减轻肝肾功能损伤和肺纤维化，值得推荐。%Objective To discuss the application of hemoperfusion combined with hemodialysis in paraquat poisoning and its curative effect .Methods The 64 cases of paraquat poisoning patients were received and cured during January 2012 and December 2015, and they were divided into experimental group (32 cases) and control group ( 32 cases ) . Experimental group was treated with hemoperfusion ( HP ) combined with hemodialysis ( HD) , and the control group was treated with HP .The curative effect , blood drug level , urea nitrogen (BUN), creatinine (Cr), Alanine Aminotransferase (ALT), tumor necrosis factor -α(TNF -α), transforming growth factor -β(TGF-β1), platelet derived growth factor (PDGF), matrix metalloproteinase-9 (MMP-9), partial pressure of oxygen (PaO2) of the two groups were recorded , Chi-square test and t test were used to analyze the data .Results The total effective rate of the experimental group was significantly higher than that of control group (P<0.05).The post-treatment blood drug level, BUN, Cr, ALT, TNF-α, TGF-β1, PDGF and MMP-9 of two groups were obviously decreased , in addition, the decrease of experimental group was huge than the control group (P<0.05).Conclusions The curative effect of HP combined with HD in paraquat poisoning is clear -cut, and it can alleviate the damage of liver and kidney function and pulmonary fibrosis , so it is worthy of promotion .
