Y 型留置针替代改良赛丁格穿刺技术在 PICC 置管中的应用

摘要

目的：探讨 Y 型留置针替代改良赛丁格穿刺技术引导在 PICC 置管中的应用及效果。方法：将110例肿瘤患者随机分为实验组58例和对照组52例，实验组采用 Y 型留置针引导 PICC 穿刺置管，对照组采用改良赛丁格穿刺针引导 PICC 穿刺置管。比较两组一次穿刺成功率、机械性静脉炎、置管后感染率、穿刺时出血量、操作时间及费用。结果：实验组一次穿刺成功率高于对照组（P ﹤0.05），机械性静脉炎发生率及穿刺费用均低于对照组（P ﹤0.05）。结论：Y 型留置针引导 PICC 穿刺置管操作方便，一次穿刺成功率高，费用低，机械性静脉炎发生率低，且不增加临床操作时间和患者出血量，值得临床推广应用。%Objective:To explore the application of Y - type indwelling needle instead of modified Seldinger puncture technique in PICC and investigate its effect. Methods:110 cancer patients were randomly divided into the experimental group(n = 58)and the control group (n = 52). Y - type indwelling needle was used in the experimental group and the modified Seldinger puncture technique was used in the control group. The one time success rate of puncture,the incidence of mechanical phlebitis,infection rate after catheterization,volume of bleeding during puncturing,operation time and cost were compared between the two groups. Results:The one time success rate of puncture was higher in the experimental group than the control group(P ﹤ 0. 05);the incidence of mechanical phlebitis and puncture costs were lower in the experimental group than the control group(P ﹤ 0. 05). Conclusion:Y - type indwelling needle used for PICC puncture has the advantages of simple operation,high one time success rate,low cost,low incidence of mechanical phlebitis,short operation time and less volume of bleeding.
