以手机 APP 服务平台进行乳腺癌术后化疗患者维护的可行性分析及临床实践

摘要

目的：探讨采用手机 APP 服务平台进行乳腺癌术后化疗患者维护的可行性，为临床实践提供依据。方法：将638例乳腺癌术后化疗患者随机分为观察组320例和对照组318例，观察组采用手机 APP 程序搭建术后维护平台，定期推送体检信息、关爱提醒、健康监测和健康教育等服务；对照组采用常规出院流程，包括出院带药使用方法及注意事项，交代家中护理注意事项，嘱定期复诊等。随访1年，比较两组健康知识掌握情况、化疗不良反应发生率、来院检测次数及用药依从性、心理状况和生活质量等指标。结果：观察组健康知识掌握率明显优于对照组（P ﹤0.05，P ﹤0.01）；观察组术后化疗不良反应发生率明显低于对照组，来院检查次数和 Morisky 评分明显高于对照组（P ﹤0.05，P ﹤0.01）；观察组抑郁、躯体功能、心理功能评分及总分高于对照组，焦虑评分低于对照组（P ﹤0.05，P ﹤0.01）。结论：基于手机 APP 服务构建乳腺癌术后化疗患者维护平台，可提高患者健康知识掌握率，降低不良反应发生率，提高患者遵医行为，改善患者心理状况和生活质量。%Objective:To explore the feasibility of using APP service platform of mobile phone for the postoperative nursing care of breast cancer patients with chemotherapy after cischarge in orcer to provice the basis for clinical practice. Methocs:638 postoperative patients un-cergoing chemotherapy were rancomly civicec into the observation group(n =320)anc the control group(n =318). The APP service plat-form of mobile phone was acoptec in the observation group to regularly provice the service like mecical information,concern,health monito-ring anc health ecucation;the conventional cischarge guicance was given to the patients in the control group. After one year of follow - up,the patients' awareness of health knowlecge,the incicence of acverse crug reaction,the number of reexamination,mecication compliance,psycho-logical status anc quality of life of the patients were comparec between the two groups. Results:The patients' awareness of health knowlecge was significantly better in the observation group than the control group(P ﹤0. 05,P ﹤0. 01);the incicence of acverse crug reaction was sig-nificantly lower anc the Morisky score was higher in the observation group than the control group(P ﹤0. 05,P ﹤0. 01);the scores of cepres-sion,somatic function anc psychological function anc the total score were higher anc the score of anxiety was lower in the observation group than the control group(P ﹤0. 05,P ﹤0. 01). Conclusion:Application of APP service platform of mobile phone to postoperative nursing care of breast cancer patients with chemotherapy after cischarge can improve the patients' awareness of health knowlecge anc recuce the incicence of acverse crug reactions,anc it can also improve patient's compliance,psychological status anc quality of life.
