氟西汀对首发迟滞性抑郁症患者血清超敏C反应蛋白水平的影响及其与病情的相关性研究

         

摘要

目的:研究氟西汀对首发迟滞性抑郁症患者血清超敏 C 反应蛋白(hs-CRP)水平的影响及其与病情的关系。方法60例首发迟滞性抑郁症患者(研究组)使用氟西汀单药治疗6周,在治疗前及疗后1、2、4、6周末进行汉密尔顿抑郁量表(HAMD-17)评定并测定血清 hs-CRP 水平。另选取60例健康对照者的血清 hs-CRP 水平。结果治疗后第6周末研究组显效率为78%。研究组治疗后第4、6周末血清 hs-CRP 水平低于治疗前(P <0.05,P <0.01),治疗后第1、2、4、6周末 HAMD 评分均低于治疗前(P <0.01);研究组治疗前及治疗后第1、2、4、6周末血清 hs-CRP 水平高于对照组(P <0.01)。相关分析显示,研究组治疗前及治疗后第1、2、4、6周末血清 hs-CRP 水平与 HAMD 评分均呈正相关(P <0.01)。结论血清 hs-CRP 可以在某种程度上反映首发迟滞性抑郁症患者病情严重程度,氟西汀对病情的改善可能与其降低血清 hs-CRP 水平有关。%Objective To explore the effect of fluoxetine on serum level of hypersensitive C-reactive protein (hs-CRP),and to explore the correlation between serum level of hs-CRP and disease severity in patients with first episode retarded depression. Methods 60 patients with first episode retarded depression (study group)were treated with fluoxetine for 6 weeks.Serum level of hs-CRP were measured and Hamilton Depression Scale-17 items (HAMD-17)were evaluated in patients in study group at baseline and at the end of the 1 st ,2nd ,4th ,6 th week of the treatment.Additionally,serum level of hs-CRP were measured in 60 healthy controls (control group).Results The response rate was 78% in study group at the end of the 6 th week of the treatment.Serum level of hs-CRP in study group decreased significantly at the end of the 4th (P <0.05),6 th week (P <0.01). Scores of HAMD-17 in study at the end of the 1 st ,2nd ,4th ,6 th week were all significantly lower than that at baseline (P <0. 01).Serum level of hs -CRP at baseline,as well as at the end of the 1 st ,2nd ,4th ,6 th week of the treatment in study group were significantly higher than that in control group (P <0.01).Correlation analysis indicated that serum level of hs-CRP at each interview-point was significantly positively correlated with score of HAMD (P <0.01 ).Conclusion Serum level of hs-CRP reflects the disease severity to some extent in patients with first episode retarded depression.The therapeutic effect of fluoxetine may be related with its effect on reducing serum level of hs-CRP.
