HIV/AIDS患者152例CD4+细胞计数与机会性感染相关分析

     

摘要

Objective To analyze the relation between CD4+ cell counts and opportunistic infections in HIV /AIDS patients and provide the reference for the treatment、 primary and secondary prophylaxis for HIV/AIDS patients with opportunistic infections. Methods According to CD4+ cell counts, 152 patients with HIV/AIDS were divided into two groups: CD4+ cell counts more than 200/μL and less than 200/μL. The total infection rate and type of 152 patients were analyzed, Differences of infection rate of opportunistic infections between two groups were compared as also. Results The total infection rate of opportunistic infections in 152 cases of HIV/AIDS patients was 93.4% , the major opportunistic infections were oral candidiasis (57.2% ), tuberculosis (50.7% ), bacterial pneumonia (32.9), sepsis (22.4%) and infectious diarrhea (21.1%). The infection incidence in the group of patients with CD4+ cell counts less than 200/μL (96%) was higher than that of patients with CD4+ cell counts more than 200/μL (80.8%). The difference was statistically significant (P < 0.05). With the decline of CD4+ cell counts, the incidence of opportunistic infections increased. Conclusions HIV/AIDS patients have a high incidence of opportunistic infections. CD4+ cell counts is an independent risk factor of opportunistic infections in HIV/AIDS patients. Therefore, CD4+ cell counts of HIV/AIDS patients should be monitored regularly to strengthen the primary and secondary prophylaxis of opportunistic infections.%目的:分析HIV/AIDS患者CD4+细胞计数与机会性感染的关系,为HIV/AIDS患者机会性感染的治疗及一、二级预防提供参考依据.方法:对2009年6月至2011年2月在我院住院的152例HIV/AIDS患者的CD4+细胞及出现的机会性感染进行分析.按患者CD4+细胞计数分为CD4+>200个/μuL与CD4+≤200个/μL两组,比较两组患者机会性感染发生率的差异.结果:152例HIV/AIDS患者机会性感染的总感染率为93.4%,主要的机会性感染是口腔念珠菌感染(57.2%)、肺结核(50.7%)、细菌性肺炎(32.9%)、败血症(22.4%)、感染性腹泻(21.1%).CD4+细胞计数≤200个/trL组患者机会性感染的发生率为96.0%,高于CD4+细胞计数>200个/μL组患者(80.8%),差异有统计学意义(P<0.05);随着CD4+细胞计数的下降,患者发生机会性感染的几率增高.结论:HIV/AIDS患者机会性感染的发生率高,CD4+细胞计数是HIV/AIDS患者发生机会性感染的独立危险因素.因此,应定期监测HIV/AIDS患者CD4+细胞计数,加强患者机会性感染的一级和二级预防.
