多排螺旋CT对肺部肿块性质诊断及鉴别诊断价值探讨

         

摘要

Objective To study and analyze the clinical value of CT in the diagnosis and differential diagnosis of pulmonary mass. Methods In the hospital from April 1 in 2012 to December 1 in 2015, 112 patients with pul-monary mass were confirmed as the research object. The patients were detected by multi slice spiral CT imaging com-bined with dynamic scanning method, Analysis of patients with CT perfusion parameters, time density curve type as well as the CT value and CT value of each period of 5 min net value added. The total diagnosis rate, sensitivity, specificity of statistical CT detection method. Results BV, BF, PS value of malignant tumor was larger than that of benign tumor, The MTT value of the benign tumor was slightly larger than that of the malignant tumor. The diagnostic threshold to PS=15 and BV=5 for malignant tumor, The accuracy, sensitivity and diagnostic accuracy were 100%, 94.7%, 98.5%. In patients with malignant tumors, 68 cases of type II, type III for 1 cases, the diagnosis rate was 98.6%. There were 30 cases of type III curve in patients with benign tumor, I, II type curves for the 3 cases, the diag-nosis rate of 97.0%. Pulmonary arterial phase of the net value of<25 Hu for 37 cases, aortic net value was greater than or equal to 25 Hu for 28 cases of, the diagnosis rate was 94.2%. Conclusion CT diagnosis and differential di-agnosis of pulmonary mass is not only economic, easy to operate, and the sensitivity, diagnostic accuracy rate is high, it is worth further promotion in lung mass .%目的:研究分析CT在肺部肿块性质诊断及鉴别诊断的临床价值。方法抽取本院2012年4月1日至2015年12月1日经确诊存在肺部肿块的患者112例作为研究对象。以多层螺旋CT成像法结合分期动态扫描法对患者进行肺部检测,分析患者CT灌注参数、时间-密度曲线类型以及5 min内各期CT值和CT净增值。统计CT检测方法的总确诊率、灵敏度、特异度。结果恶性肿块的血容量(BV)、血流量(BF)、表面通透性(PS)值均大于良性肿块,而良性肿块的平均通过时间(MTT)值则稍大于恶性肿块。以PS≥15且BV≥5为恶性肿瘤的诊断阈值,其特意度、灵敏度、诊断准确率分别为100%、94.7%、98.5%。恶性肿块患者中Ⅰ、Ⅱ型曲线为68例,Ⅲ型为1例,确诊率为98.6%。良性肿块患者中Ⅲ型曲线有30例,Ⅰ、Ⅱ型曲线为3例,其确诊率为97.0%。肺动脉期净增值<25 Hu的为37例,主动脉期净增值≥25 Hu的为28例,确诊率为94.2%。结论 CT诊断及鉴别诊断肺部肿块不仅经济、操作简单,且灵敏度、诊断准确率均高,值得在肺部肿块中进一步推广。
