Stroop色词测验评估乙型肝炎肝硬化患者认知功能

摘要

Objective To evaluate the cognitive functions in patients with compensated hepatitis B liver cirrhosis.Method Stroop color-word test (CWT) was conducted in 60 patients with compensated hepatitis B liver cirrhosis and in 60 healthy controls.Results The time of CWT-A (25.01±5.16s)in liver cirrhosis patients and in healthy controls (28.83±4.76s) were not significantly different (9＞0.05);The time of CWT-B (42.01±11.15s) and CWT-C(78.12±23.74s)in liver cirrhosis patients were longer than those in healthy controls(CWT-B:42.01±11.15s,CWT-C:78.12±23.74s,respectively,P＜0.05) ; The accuracy of CWT-A (49.33±0.88),CWT-B (47.73±2.25) and CWTC (45.69-±4.90) in liver cirrhosis patients were more greater than those in healthy controls (CWT-A:49.63±0.55,CWT-B:48.73 ±1.29,and CWT-C:47.13 ±2.24,respectively,P＜0.05) ; Multiple stepwise regression analysis showed that age,education and serum total bilirubin levels entered the regression equations of the time of CWT-A;and age,education and gender entered the regression equations of the time of CWT-B.Conclusion The patients with compensated hepatitis B liver cirrhosis had impairment in focused attention and response inhibition,and their age,education,gender and liver functions might be related to the focused attention.%目的 评估肝功能稳定的乙型肝炎肝硬化患者的认知功能.方法 采用Stroop色词测验(CWT)法对60例肝功能稳定的乙型肝炎肝硬化患者和60例健康对照者进行测评和比较.结果 乙型肝炎肝硬化患者CWT-A耗时(25.01±5.16s)与健康组(28.83±4.76s)差异无统计学意义(P＞0.05)；乙型肝炎肝硬化患者CWT-B耗时(42.01±11.15s)和CWT-C耗时(78.12±23.74s)均长于健康组(分别为38.14±9.58s和68.61±16.17s,P＜0.05)；乙型肝炎肝硬化患者CWT-A 、CWT-B和CWT-C的正确数分别为49.33±0.88个、47.73±2.25个和45.69±4.90个,均低于健康组(分别为49.63±0.55个、48.73±1.29个和47.13±2.24个,P＜0.05)；多元逐步回归分析显示,CWT-A耗时与患者的年龄、文化程度、总胆红素水平相关(R2=0.39,P＜0.001),CWT-B耗时与患者的年龄、文化程度和性别相关(R2=0.29,P＜ 0.001),未发现患者的人口学或疾病特征与CWT-C耗时相关.结论 肝功能稳定的乙型肝炎肝硬化患者在集中注意和反应抑制方面存在不同程度的障碍,年龄、文化程度、性别和总胆红素水平是集中注意的影响因素.
