目的 探讨腓动脉终末穿支为蒂的腓肠神经岛状皮瓣对足踝软组织缺损的临床修复价值.方法2012年8月-2015年8月,14例足踝外伤患者,经清创、VSD治疗7 d,待创面感染控制后,采用腓动脉终末穿支供血的腓肠神经岛状皮瓣修复足踝创面,供区直接封闭或游离植皮.结果所有皮瓣均成活,1例术后2 d出现皮瓣远端静脉回流障碍,拆除部分缝线,经换药后愈合.术后随访6个月~2年,皮瓣质地优良,无明显臃肿,无瘢痕挛缩.结论腓动脉终末穿支供血的腓肠神经岛状皮瓣切取简单,成活率高,是修复足踝皮肤软组织缺损的良好术式.%Objective To explore the clinical value of sural neurofasciocutaneous flap pedicled with terminal perforator of the peroneal artery for soft tissue defect of the foot and ankle. Methods From August 2012 to August 2015, 14 cases of foot and ankle trauma patients, after debridement, VSD vacuum drainage for 7 days, until wound infection was controled, were treated by the sural nerve island flap with peroneal artery terminal perforation to repair foot and ankle wounds, the donor area had direct closure or free skin graft. Results All the flaps survived. One patient had distal venous return obstruction at 2 days after operation. Some of the sutures were removed and healed after dressing. The flaps were followed up for 6 months to 2 years. The skin flaps texture was excellent, was not overstaffed was fine, No scar contracture. Conclusion The sural neurocutaneous island flap with peroneal artery terminal perforator is simple and has high survival rate. It is a good procedure for repairing soft tissue defect of foot and ankle.
