富血小板纤维蛋白(PRF)在骨折术后内置物外露治疗中的临床应用

         

摘要

目的 探讨富血小板纤维蛋白(Platelet-rich fibrinPRF)在骨折术后深部组织及内置物外露创面中覆盖创面的临床疗效.方法 2015年7月-2016年6月,对11例骨折后切口感染、裂开导致钢板及骨外露的患者,未取出钢板,在给予清创后应用PRF填充创腔,部分创面后期结合植皮治疗.结果 11例中4例创面自行愈合,7例后期经植皮治疗覆盖创面.术后随访3~6个月,创面均愈合良好,无破溃及窦道形成,X线片显示原骨折部位骨痂形成良好或骨折已愈合.结论 应用富血小板纤维蛋白治疗创伤后深部组织外露,可以避免取出内置物,减少二次损伤、骨折不愈合及畸形愈合的风险,效果满意.%Objective To explore the clinical curative effect of PRF in treating internal implant exposure after fracture surgery. Methods From July 2015 to June 2016, 11 patients who had incision infection, disruption, leading to bone and steel plate exposure, were treated by the filling traumatic cavity with PRF after debridement, without removal of stall plate, some patients were treated combined with skin trans plantation. Results 4 cases in 11 patients, and 7 cases obtained wound the cover with PRF by skin graft. Patients were followed up 3 to 6 months after operation, all of the wounds healed well, no ulceration and sinus formed, X-ray taken postoperatively revealed or bones union. Conclusion Making full use of PRF on deep tissue exposure of fracture surgery can avoid to remove implant and reduce the risks of second injury, fracture nonunion or malunion, and the result was satisfactory.
