目的 观察抗生素盐水冲洗治疗手部骨关节感染的临床疗效.方法 2013年2月-2016年2月,对30例手部骨关节感染的患者,采用静脉滴注抗生素的同时,清除感染病灶、放置冲洗管及引流管、关闭创面、负压引流,术后抗生素盐水冲洗治疗,并进行回顾性分析.结果 30例均获随访,随访时间3~24个月,平均6个月.28例创口Ⅰ期愈合,2例创口皮肤愈合差,经过2~3周换药后愈合;并发引流管脱落1例,予以再次手术缝合固定;4例骨不愈合,Ⅱ期植骨后愈合.创面均未再次出现皮肤软组织、骨关节感染以及创面不愈合等并发症.结论 在全身抗生素应用的基础上,经过彻底清创,结合局部抗生素盐水冲洗治疗手部骨关节感染,能有效控制、消除骨关节内感染病灶,操作简便、并发症少,是一种行之有效的治疗方法.%Objective To observe the clinical effect of the treatment of bone and joint infections with antibiotics saline washing. Methods From February 2013 to February 2016, 30 cases with hand bone and joint infection were retrospectively analyzed, used by intravenous infusion antibiotics, at the same time removed the infected lesions, and placed the douche tube and drainage tube, and closed the wound and drainage, postoperative antibiotic saline irrigation treatment. Results All the 30 patients were followed up for 3 to 24 months with an average of 6 months. 28 cases of wound healed, 2 cases of skin wound healing bad, healed after 2~3 weeks of dressing. Concurrent drainage pipe falled off in 1 case, was reoperation. 4 cases of nonunion, healed after second stage bone grafting. There were no complications such as skin soft tissue, bone joint infection and wound healing. Conclusion On the basis of antibiotics in general, after thorough cleaning, combined with topical antibiotic saline washing in the treatment of hand bone and joint infection. It can effective control and eliminate bone and joint infection focus, simple operation, less complications, and it is an effective treatment method.
