目的 探讨适合老年患者术后疼痛的评估工具.方法 选取108例老年手术患者,采用词语描述评估工具(VDS)、数字评估工具(NRS)、视觉模拟评估工具(VAS)和五指法评估工具(FFS)作为评价工具,分别让患者选择其中最喜欢的一种评估工具,向患者解释工具的使用后计算作答时间、解释次数和术后4h疼痛程度的首次应答成功率.结果 患者对FFS、VDS、VAS、NRS的首选率分别是70例(64.8％)、22例(20.4％)、8例(7.4％)和8例(7.4％),解释作答时间分别为(15.11±1.50)s、(20.55±1.90)s、(30.22±1.86)s和(34.78±1.48)s;反复解释次数分别为(1.08±0.04)次、(1.15±0.10)次、(1.44±0.18)次和(1.89±0.11)次(均P＜0.01);术后4h患者首次应答成功率FFS、VDS、VAS和NRS分别为98.6％、90.9％、62.5％和75.0％(P＜0.01).结论 术后老年患者对FFS首选率最高,作答时间最短,解释次数最少,术后4h疼痛评估首次应答成功率最高,可作为评估老年术后患者疼痛强度的首选评估工具.%Objective To explore a suitable tool assessing postoperative pain for elderly patients.Methods Totally 108 elderly surgery patients were selected,and Verbal Descriptor Scale (VDS),Numeric Rating Scale (NRS),Visual Analogue Scale (VAS),and Five Finger Scale (FFS) were used as evaluation tools.The patients were asked to choose a favorite assessment tool,and were explained utilization of each tool,then the time of answering,the times of interpretations,the success rate of first response to pain severity 4 hours after operation were calculated.Results There were 70(64.8％),22(20.4％),8(7.4％) and 8(7.4％) patients selecting FFS,VDS,VAS and NRS firstly,the time consuming of explaining and answering were(15.11± 1.50)s,(20.55 ± 1.90)s,(30.22±1.86)s and (34.78±1.48)s respectively,and repeated explanation were (1.08±0.04) 、(1.15±0.10) 、(1.44±0.18)and (1.89±0.11) times respectively (P＜0.01 for all).The success rate of first response to pain severity 4 hours after operation for FFS,VDS,VAS and NRS was 98.6％,90.9％,62.5％ and 75.0％ respectively (P＜0.01).Conclusion Most postoperative elderly patients select FFS,its answering time is shortest,interpretation times is least,and its success rate of first response to pain severity 4 hours after operation is highest,thus it can be used as the first tool to assess pain severity for elderly postoperative patients.
