Objective To study the ways of shortening fasting drink time for infant with critical congenital heart disease. Method 556 infants with critical congenital heart disease was randomly divided into two groups, 264 patients in control group, 292 patients in experimental group. Patients in control group was given traditional fasting drink scheme, include fasting eight hours and drinking 6 hours before anesthesia. Patients in experimental group was given conventional fast 4～6 hours and drinking 2 hours. The indicators, including thirsty, sense of hungry, fatigue, crying, vomiting and aspiration, body temperature, oxygen attack and heart failure, gastric liquid pH and blood sugar, was observed and compared between the two groups. Result The incidence of thirsty, hungry, fatigue, crying,body temperature more than 38. 0 ℃ , oxygen attack and heart failure in experimental group was significantly lower than that of in control group(P<0. 05). There was significant difference in blood sugar level between two groups (P<0.05). There was no significantly gastric pH, vomiting and aspiration during operation between the two groups (P>0. 05). Conclusion Preoperative fast 4～6 hours and the drinking 2 hours for selective surgery infant with critical congenital heart and disease was safe. It didn't increase gastric pH level, vomiting and aspiration during operation, and can reduce adverse reactions.%目的 探讨缩短危重先天性心脏病小婴儿术前禁食禁饮时间.方法 将择期手术的556例危重先天性心脏病小婴儿随机分为两组,对照组264例,实验组292例.对照组采用传统的禁食、禁饮方案,麻醉前常规禁食8h,禁饮6h.实验组术前常规禁食4～6 h,禁饮2h,观察比较两组患儿的口渴、饥饿感、疲乏无力、哭闹、术中呕吐及误吸、体温、缺氧发作和心力衰竭的发生率,测量胃液pH值及血糖值.结果 与对照组比较,实验组的口渴、饥饿感、疲乏无力程度、哭闹、体温≥38.0℃、缺氧发作和心力衰竭的发生率均显著低于对照组,差异有显著意义(P＜0.05)；两组血糖值的差异也有显著意义(P＜0.05)；胃液pH值差异无显著意义(P＞0.05)；实验组和对照组术中呕吐及误吸比例差异无显著意义(P＞0.05).结论 择期手术的危重先天性心脏病小婴儿术前禁食4～6 h和禁饮2h是安全的,没有增加胃液pH值、术中呕吐及误吸的发生率,同时可降低禁饮、禁食所引起的不良反应.
