老年2型糖尿病患者低血糖发生时间分析

摘要

①目的观察老年2型糖尿病患者低血糖发生的时间，分析其发生的规律。②方法回顾分析郑州大学第一附属医院老年内分泌科2015年4月～2016年4月127例老年2型糖尿病患者在院期间低血糖发生的时间，分析其与昼夜、月份、季节的关系。③结果按季节分析，老年2型糖尿病患者春季（3～5月）发生低血糖概率最大，冬季（12～2月）次之，夏季（6～8月）概率最低；按月份分析，3月发生低血糖的可能性最大，低血糖的发生在 3月及9月左右出现两个高峰阶段，且 3月的可能性大于9月；按24小时制时间分析，清晨空腹（5：00～8：00）发生低血糖所占的比例最大，午餐前（11：00～14：00）次之，三餐后发生低血糖风险较小，采用 Rayleigh′s 检验计算雷氏 Z＝12．4383＞Z0．05＝2．9957，差异有统计学意义。④结论老年2型糖尿病患者一年之中易于春季、一日之中易于清晨发生低血糖，血糖变化节律随衰老有异于中青年人的趋势，但目前这种变化趋势尚不明确，故对老年2型糖尿病患者的管理需要医护人员对其进行系统化、专业化的教育和指导，终身配合治疗。%Objective To observe the occurrence time of hypoglycemia in elderly patients with type 2 diabetes,and to analyze its rule.Methods Reviewed and statisticed the occurrence time of hypo-glycemia in elderly patients with type 2 diabetes of the department of geriatrics endocrinology,the first affiliated hospital of Zhengzhou University,in April 2015 to April 2016,a total of 127 cases during their hospitalization,and analyze the relationship among the season,month,day and night.Results Accord-ing to the season,spring was the most likely one to appear hypoglycemia,the second was the winter, and the summer was the least;According to the month,March was the most possibly one of hypoglyce-mia.The appearance of hypoglycemia had two peak stage on the March and the September,and the peak of March was higher;According to the time,early in the morning on an empty stomach(5:00-8:00)had the largest percentage of hypoglycemia,the second was before lunch (11 PM)times,and there was less risk of hypoglycemia occurred after meals,the Z value obtained by using the Rayleigh's in-spection=12.4383>Z0.05 =2.9957,said that the difference was statistically significant.Conclusion In elderly patients with type 2 diabetes,the hypoglycemia is more easy to appear in the spring of year,in early morning of day.The rhythm of blood glucose changes with aging,and is different from the trend of young people,but at present the change trend is not clear.Therefore,doctors have to make the sys-tematic and specialized education and guidance of elderly patients with type 2 diabetes,and give them the lifelong treatment.
