卵泡液中FSH、LH水平与IVF/ICSI-ET结局的关系

Objective To investigate the relationship between follicular fluid follicle-stimulating hormone(FSH) and luteal hormone(LH) levels and in vitro fertilization/intracytoplasmic sperm injection-embryo transfer(IVF/ICSI-ET) outcome.Methods The was obtained on the oocyte retrieval day from 111 patients undergoing IVF/ICSI-ET therapy.Enzyme linked immunosorbent assay(ELISA) was used to determine the levels of FSH and LH in follicular fluid.The transplanted patients were divided into pregnant group and non-pregnant group.All samples were divided into three groups according to the levels of FSH in follicular fluid:group A(FSH<3.0 U·L-1),group B(FSH 3.0~7.5 U·L-1) ,and group C(FSH>7.5 U·L-1).The relationship between follicular fluid FSH and LH levels and IVF/ICSI-ET outcome was analyzed.Results The levels of FSH and LH in follicular fluid were positively correlated with the dosage of gonadotropin for each competent follicle(r=0.396 and 0.191,respectively;P<0.05),negatively correlated with the number of oocytes retrieved(r=-0.401 and-0.237,respectively;P<0.05),but not correlated with the fertilization rate and high quality embryo rate(P>0.05).There were significant differences between pregnant group and non-pregnant group in fertilization rate,high quality embryo rate and FSH levels in follicular fluid(P<0.01),but not in other indicators(P>0.05).The high quality embryo rate and pregnancy rate in group B were statistically higher than those in group A and group C(P<0.05).Furthermore,the fertilization rate in group B was significantly higher than that in group C(P<0.05).Conclusion Follicular fluid levels of FSH,but not LH,are correlated with IVF/ICSI-ET outcome,and the optimal outcome is achieved at FSH concentration of 3.0-7.5 U·L-1.%目的 探讨卵泡液中卵泡刺激素(FSH)和黄体生成素(LH)水平与体外受精/卵胞质内单精子显微注射-胚胎移植(IVF/ICSI-ET)结局的关系.方法 选择接受常规长方案IVF/ICSI-ET治疗的111例不孕患者,并收集取卵日卵泡液,采用酶联免疫吸附试验(ELISA)检测其中FSH和LH的水平.将移植的患者按是否获得妊娠分为妊娠组与未妊娠组.所有样本按卵泡液FSH水平分为FSH<3.0 U·L-1组(A组)、FSH3.0~7.5 U·L-1组(B组)、FSH>7.5 U·L-1组(C组).分析卵泡液FSH、LH水平与IVF/ICSI-ET结局各项指标的关系.结果 卵泡液中FSH、LH水平与每个有效卵泡Gn用量呈正相关(r=0.396、0.191,P<0.05),与获卵数呈负相关(r=-0.401、-0.237,P<0.05),与受精率、优质胚胎率并无相关性(P>0.05). 妊娠组与未妊娠组的受精率、优质胚胎率及卵泡液FSH水平存在显著差异(P<0.01),而其他指标2组比较差异无统计学意义(P>0.05).B组优质胚胎率、妊娠率均显著高于A组、C组(P<0.05),受精率也高于C组(P<0.05).结论 卵泡液LH水平与IVF/ICSI-ET结局无相关性.卵泡液FSH水平与IVF/ICSI-ET结局相关,浓度在3.0~7.5 U·L-1时IVF/ICSI-ET结局最佳.
