经尿道钬激光治疗非肌层浸润性膀胱肿瘤的临床疗效分析

摘要

目的 比较经尿道钬激光膀胱肿瘤切除术和经尿道膀胱肿瘤电切术治疗非肌层浸润性膀胱癌的疗效和安全性.方法 回顾性分析我院于2011年10月到2013年1月收治行手术治疗的71例非肌层浸润性膀胱癌患者的临床资料,钬激光组31例和电切组40例.比较两组患者手术时间、膀胱冲洗率、尿管留置时间、住院时间、手术并发症以及术后复发率等指标.结果 钬激光组手术时间、膀胱冲洗率、尿管留置时间、住院时间及膀胱穿孔率明显低于电切组,差异有统计学意义( P<0.05 ).随访24~37月,中位31月,两组患者2年累积复发率分别为19.35% 和27.50%,钬激光组复发率明显低于电切组,差异有统计学意义( P<0.05 ).结论 钬激光治疗非肌层浸润性膀胱癌疗效显著,安全可靠,并发症少,复发率低,值得临床推广使用.%Objective To compare the safety and efficacy of holmium laser and transurethral resection of bladder tumors (TURBT) for treating non-muscle invasive bladder cancer (NMIBC).Methods Clinical data of 71 patients with NMIBC treated during Nov.2011 and Jan.2013 were retrospectively analyzed.There were 31 cases which underwent transurethral hol-mium laser therapy and 40 which received TURBT.The operation time,bladder irrigation rate,catheterization time,hospital stay,complications,and the rate of tumor recurrence between the two groups were compared.Results The mean operation time,bladder irrigation rate,catheterization time,hospital stay of the holmium laser group were statistically shorter or lower compared with those of the TURBT group.During the follow-up of 24-37 months (median 31 months),the 2-year cumulative recurrence rate of the holmium laser and TURBT group was 19.35% and 27.50%,respectively.There was significant differ-ence between the two groups.Conclusions Compared with TURBT,transurethral holmium laser therapy is more safe and ef-fective with fewer complications for NMIBC.
