肾上腺支气管源性囊肿诊治分析（附3例报告）

摘要

目的：探讨肾上腺支气管源性囊肿的诊治。方法回顾性分析2004年11月至2014年10月本院收治的3例肾上腺支气管源性囊肿患者的临床资料：3例患者中男2例，女1例；年龄36～45岁，均无特殊临床表现，囊肿部位均为左侧。例1肾上腺占位是在检查其他疾病时偶然发现；例2及例3则为体检发现。3例均常规行肾上腺内分泌检查，包括相应的嗜铬细胞瘤或副神经节瘤的相关检查，此外例1行M RI及I131‐M IBG显像，例2及例3行C T及生长抑素受体显像。结果3例均成功行腹腔镜左肾上腺肿物切除术，术后恢复顺利。术后病理均回报为符合支气管源性囊肿。结论肾上腺支气管源性囊肿临床罕见，影像学检查有时难以明确其囊性特征，需与腹膜后其他肿物进行鉴别，确诊通常依赖于术后病理。治疗以腹腔镜肾上腺肿物切除术为首选。%ABSTRACT:Objective To investigate the diagnosis and treatment of adrenal bronchogenic cyst .Methods Clinical data of 3 cases of adrenal bronchogenic cyst admitted in our hospital during Nov .2004 to Oct .2014 were retrospectively reviewed . The patients included 2 males and 1 female ,aged 36 to 45 years old .No patients had specific symptoms ,and all cysts were lo‐cated in the left side .The adrenal placeholder of case 1 was found incidentally in medical check for other disease ,and the other 2 cases were discovered in routine health check‐up .All patients received endocrinological evaluation ,including pheochromocy‐toma or paraganglioma related inspection .MRI and I131‐MIBG imaging were performed in case 1 ,CT scan and somatostatin re‐ceptor imaging were performed in another 2 cases .Results All patients received left laparoscopic adrenal neoplasm resection successfully with uneventful recovery ,and the pathological examination revealed bronchogenic cyst .Conclusions Adrenal bronchogenic cyst is extremely rare and difficult to confirm .Differentiating it from other retroperitoneal neoplasms is necessa‐ry .So far diagnosis still depends on the pathology ,and laparoscopic adrenal neoplasm resection is the first choice of treatment .
