头颈肩真空固定袋与透明头枕两种固定装置的摆位误差对比分析

摘要

目的:对比头颈肩真空固定袋和透明头枕在头颈部肿瘤三维适形及调强放射治疗中固定体位时的摆位误差.方法:将2008年至2009年的调强放疗患者随机选取200人分为两组,每组100例,均使用头颈肩热塑膜固定体位.第一组患者头枕头颈肩真空固定袋,第二组患者头枕透明头枕.每例患者第一次治疗前均在加速器上拍摄正 、侧位等中心验证片各一张,与治疗计划中的DRR片比较,分别计算出X、Y、Z方向的摆位误差.结果:头颈肩真空固定袋组X、Y、Z方向平均误差分别为:X(0.1±0.68902)mm,Y(-0.03±0.73106)mm,Z(-0.99±1.08707)mm,透明头枕组X、Y、Z方向平均误差分别为:X1(-0.16±0.78779)mm、Y1(0.06±1.09931)mm、Z1(-0.4±1.12815)mm.其中头颈肩真空固定袋组X、Z方向的误差均明显小于透明头枕组(P<0.05),Y方向误差无统计学意义.结论:使用头颈肩真空袋可明显降低头颈部肿瘤三维适形及调强放射治疗的摆位误差.%Objective: To study the error of neck shoulder vacuum fixed bag and transparent head rest when the pate tumor treated by three dimensional suitable shape and adjusts in the strong radiotherapy the fixed body posture pendulum position error.Methods: Total of 200 patients with strong radiotherapy were divided into two groups 100 in each gruop.First group of patient used head rest neck shoulder vacuum fixed bag, second group of patient head rest transparent head rest.Before first treatment, photographs, central confirmation piece and side position on the accelerator,compares with the treatment plan's in DRR piece,and calculated X, Y, Z direction pendulum position error.Results: Neck shoulder vacuum -bag group X, Y, the Z direction average error X ( 0.1 ± 0.68902 ) mm, Y( -0.03 ±0.73106 )mm , Z( -0.99 ± 1.08707 )mm , transparent head rest group: X, Y, Z direction average error:X1 ( -0.16 ±0.78779 )mm, Y1( 0.06 ± 1.09931 )mm,Z1( -0.4 ± 1.12815 )mm.And the neck shoulder vacuum -bag group X, Z direction's error was obviously smaller than the transparent head rest group ( P ＜ 0.05 ), Y lateral error was not significant.Conclusion: The use of neck shoulder vacuum - bag may reduce the neck department tumor three dimensional suitable shape obviously and adjust the strong radiotherapy pendulum position error.
