Objective:To discuss the optimal time of wearing permanent prosthesis and to assess speech intelligi-bility of permanent prosthesis.Methods:18 patients were treated by total resection of the maxilla in the maxillary si-nus and survived over 1 year.13 patients converted CAD/CAM prostheses to moderate-low pure titanium permanent prostheses 6 months after surgery,the remaining 5 patients did not require replacement.After 6 months and 1 year postoperative observation of speech intelligibility and facial features were compared.SPSS 13.0 software to do statisti-cal analysis.Results:Speech intelligibility of 13 patients with immediate prosthesis after 6 months surgery was(91.21 ± 1.13)%,and with permanent prosthesis was(95.00 ± 1.78)%.By t-test,P value <0.05 was considered sta-tistically significant.The facial change of permanent prosthesis compared with immediate prosthesis showed no statistical significance(P>0.05).Conclusion:CAD/CAM and permanent prosthesis can effectively close oro-antral commu-nication and well reconstruct the shape of oral cavity to elevate the score of speech intelligibility and improve voice function.Permanent prosthesis can obtain higher satisfactory levels of patients.%目的:探讨佩戴永久性赝复体的时机,评价永久性赝复体的语音修复效果.方法:病例来源于中国医科大学附属第一医院耳鼻咽喉科自2012年10月至2015年3月收治的上颌窦癌行上颌骨全切术后生存1年以上赝复体18例,13例在术后6个月重新取模更换永久性赝复体,其余5例不要求更换.术后6个月和术后1年分别进行语音清晰度、面容方面对比观察.采用SPSS 13.0软件做统计学处理.结果:13例患者术后6个月佩戴即刻赝复体语音清晰度为(91.21 ± 1.13)%,术后6个月全部更换永久性赝复体,佩戴后语音清晰度在(95.00 ± 1.78)%,经t检验,P<0.05.术后1年13例更换永久性赝复体的患者面容方面和6个月中空充填式赝复体比较无统计学意义(P>0.05).结论:上颌窦癌行上颌骨全切除后6个月面容及术腔愈合稳定,可更换永久性赝复体.重新取模制作的纯钛永久性赝复体较中空充填式即刻赝复体,使术腔封闭更紧密、重量更轻、患者感觉美观舒适,语言清晰度更清晰,获得较高的患者满意度.
