中老年2型糖尿病患者社区康复的生活质量及其相关因素分析

摘要

Objective To master the quality of life among the middle age and elderly patients with type 2 diabetes mellitus (T2DM) and to investigate its influencing factors. Methods 162 cases of T2DM rehabilitated in Nanzhuang town community were selected and performed the evaluation on the quality of life by using the SF-36 scale and the diabetes specific quality of life (DSQL) scale. The obtained data were statistically analyzed. Results The quality of life in the middle age and elderly patients with T2DM was generally declined. The damage in the overall health dimension of SF-36 and psychological/mental dimension of DSQL was maximal. The scores of SF-36 were generally lower than those in the norm of 6 provinces and cities (Jilin,Beijing, Lanzhou,Chongqing,Suzhou,Wuhan),while the scores of DSQL were superior to those in the domestic norm,the differences were statistically significant(P<0.01);The taste,eating fruit time,health knowledge,medical assurance satisfaction,dietary control,milk and soytbean milk,poor sleep and monthly burden cost were the changeable factors influencing the quality of life. Conclusion The quality of life among the middle age and elderly patients with T2DM rehabilitated in the community is decreased. In the com-munity standardized management,the comprehensive measures are adopted to promote the influencing factors conducive to change of the quality of life,thus which improves the quality of life.%目的：掌握在社区康复的中老年2型糖尿病患者的生活质量，探讨其影响因素。方法选择2014年南庄镇社区康复的162例2型糖尿病患者，应用健康调查表（SF-36量表）、糖尿病患者生存质量特异性量表（DSQL）评价生活质量，所得数据进行统计分析。结果中老年2型糖尿病患者的生活质量普遍降低，SF-36的总体健康维度、DSQL的心理/精神维度受损最大，SF-36总体上差于6省市常模（吉林、北京、兰州、重庆、苏州、武汉），而DSQL则全面优于国内常模，差异有统计学意义（P＜0.01）；口味、进食水果时间、健康知识、医保满意、控制饮食、牛奶豆浆、睡眠不好、月负担费用等是影响生活质量可改变的因素。结论社区康复的中老年2型糖尿病患者生活质量降低，在社区规范管理中采取综合措施，促进影响因素向有利于生活质量转变，从而提高其生活质量。
