FeNO在儿童慢性气道炎症疾病诊断中的应用

             

摘要

Objective: To analyze the diagnostic value of exhaled nitric oxide (FeNO) in children with chronic airway inflammation. Methods: 399 children with asthma in our hospital from June 2015 to December 2017 were divided into acute asthma group (group A1), chronic asthma group (group A2), asthma clinical remission group (group A3), atypical asthma group (group B1), other causes induced cough group (group B2), recurrent asthma group with the history of idiopathic symptoms (group C1) and relapse without the history of idiopathic symptoms (group C2) and pneumonia group (group D). The changes of FeNO, EOS counts and total IgE levels in each group were compared. Results: FeNO values in groups A1, A2, A3, B1 and C1 were significantly higher than those in groups B2, C2 and D (P < 0. 01), and during the onset period of asthma it was the highest.With the improvement of clinical symptoms, the level of FeNO decreased, and during the remission period of asthma it was still at a high level. However, EOS count and serum IgE levels increased in all groups, but there was no significant difference among the groups (P> 0. 05). Conclusion: As an index of airway inflammation, FeNO can better reflect the degree of airway EOS inflammation and can be used for the diagnosis and differential diagnosis of chronic airway inflammation in children. The combined detection of peripheral blood EOS count, serum total IgE and FeNO can not only reflect the body's allergic state, but also assess the degree of airway inflammation, and can more comprehensively assess the disease.%目的:分析呼出气一氧化氮(FeNO)在儿童慢性气道炎症疾病中的诊断价值.方法:选择2015年6月至2017年12月我院患儿399例为研究对象,分为哮喘急性发作期组(A1组)、哮喘慢性持续期组(A2组)、哮喘临床缓解期组(A3组)、不典型哮喘组(B1组)、其他原因咳嗽组(B2组)、有特应征病史的复发性喘息组(C1组)、无特应征病史的复发性喘息组(C2组)及肺炎组(D组),比较各组FeNO、EOS计数及总IgE水平的变化.结果:A1、A2、A3、B1组和C1组患儿FeNO值显著高于B2组、C2组及D组(P <0.01),且发作期最高,随疾病临床症状的改善,FeNO水平下降,哮喘缓解期仍处于较高水平.而EOS计数及血总IgE水平在各组患儿均增高,但各组间无显著性差异(P> 0.05).结论:FeNO作为气道炎症指标,能更好地反映气道EOS性炎症程度,可用于小儿慢性气道炎症性疾病的诊断及鉴别诊断.联合检测外周血EOS计数、血清总IgE和FeNO,既能反映机体的过敏状态,又能评估气道炎症程度,能更全面地对疾病进行评估.
