蛋白芯片联合检测病原体抗体在急性呼吸道感染中诊断价值的探讨

Objective :Research of diagnostic value in Protein Chip for combined detection of pathogen antibodies in pa-tients with acute respiratory infections ,provide effective test data for the clinical diagnosis .Methods:Collect diagnostic serum specimen of 474 ARI cases of children with ARI .Application of Protein Chip technique to detect antibody among the common pathogens ,including mycoplasma pneumoniae(MP) ,adenovirus(ADV) ,influenza virus(FLU) ,respiratory syncytial virus(RSV) .Results:440 positive pathogens cases were tested in 474 children ,99 .2% had evidence of patho-gens infection ,the positive rates of age < 1 year ,1≤ y< 3 ,3≤ y ≤6 ,> 6 years old were 86 .8% ,94 .0% ,97 .0% , 96.9% .Positive rates of different age groups were significantly different (P<0 .05);different pathogens among all age groups were significantly different (P<0 .05);among the 4 pathogens ,the positive rate of FLU was the highest ,fol-lowed by MP ,RSV and ADV .226 (47 .7% ) cases of them were mixed infection ,which were infected by two and more pathogens .Conclusion:Protein chip high positive rate of children with acute respiratory infection ,the main pathogens causing acute respiratory infection in children are FLU ,MP and RSV .The mixed infection is more widespread ,there are significant differences in different age groups.%目的：采用蛋白芯片技术联合检测急性呼吸道感染（Acute respiratory infection ，ARI）患儿病原体，探讨蛋白芯片在急性呼吸道感染疾病中的诊断价值，为临床病原学诊断提供依据。方法：收集诊断A RI患儿的血清标本474例，应用蛋白芯片法检测其中常见病原体抗体，包括肺炎支原体（Mycoplasma pneumonia ，MP）、腺病毒（Adenovirus ， ADV）、流感病毒（Influenza virus ，FLU）、呼吸道合胞病毒（Respiratory syncytial virus ，RSV）。结果：474例患儿中抗体阳性470例，阳性率99．2％，其中根据患儿年龄分为四组，第一组＜1岁，第二组1～3岁，第三组3～6岁，第四组＞6岁，各组阳性率分别是86．8％，94．0％，97．0％，96．9％比较各组差异有统计学意义（ P＜0．05）；各年龄组不同病原体感染差异有统计学意义（P＜0．05）；四种病原体中 FLU 阳性率最高，其余依次为MP、RSV、ADV。混合感染226例，占47．7％，主要是两种及以上病原体混合感染。结论：蛋白芯片检测儿童急性呼吸道感染病原体阳性率高，其中阳性率较高的病原体为FLU、MP和RSV ，混合感染比较普通，各年龄组感染的病原体有差异。
