高通量血液透析对尿毒症患者生存质量影响分析

         

摘要

目的::探讨高通量血液透析对尿毒症患者生存质量的影响。方法:选取医院行持续血液透析治疗的尿毒症患者70例作为研究对象,采用数字随机表将患者随机分为观察组和对照组,观察组予以高通量血液透析治疗,对照组予以低通量血液透析治疗,检查两组治疗前、后微炎症状态和营养状态,随访1年,记录两组生存率。结果:观察组血液透析后 CRP、TNF-α、P、PCT 分别为(5.96±2.06)mg/L、(1.84±1.03)μg/L、(1.29±0.41)mmol/L、(1.43±0.62) ng/ml 均明显下降,与对照组透析后相比,差异显著,具有统计学意义(P <0.05);观察组透析后 BMI、ALB、TP 分别为(23.27±0.71)、(35.96±2.07)g/L、(66.01±1.63)g/L 均上升,与对照组透析后相比,差异显著,具有统计学意义(P <0.05)。随访1年,两组生存率无明显差异(P >0.05)。结论:高通量透析和低通量透析患者的近期生存率无差异,但高通量透析治疗后能够改善微炎症状态,提高免疫力,有利于提高患者生存质量。%Objective:To explore the effects of high-flux hemodialysis on quality of life of patients with uremia.Meth-ods:70 uremic patients admitted into the hospital for continuous hemodialysis therapy were selected as the research ob-jects and were randomly divided into the observation group and the control group by the random number table.The ob-servation group was treated with high-flux hemodialysis therapy while the control group with low-flux hemodialysis therapy.The micro inflammatory and nutritional status of the two groups before and after the treatment were exam-ined.With a year of follow-up,the survival rates of the two groups were recorded.Results:CRP,TNF-α,P,PCT of the observation group after hemodialysis respectively were (5.96±2.06)mg/L,(1.84 ± 1.03)μg/L,(1.29 ±0.41 ) mmol/L and (1.43 ± 0.62 )ng/ml which decreased significantly,compared with those of the control group and the difference was obvious and of statistical significance(P <0.05).After the dialysis,BMI,ALB and TP of the observa-tion group respectively were (23.27±0.71),(35.96±2.07)g/L and (66.01 ±1.63)g/L which increased significant-ly,compared with those of the control group and the difference was obvious and of statistical significance (P <0.05). During 1 year of follow-up,there was no significant difference in the survival rates of the two groups (P >0.05).Con-clusion:There were no obvious differences in recent survival rates of patients with high-flux dialysis and low-flux hemo-dialysis,but the high-flux hemodialysis can improve the micro-inflammatory and immunity and is conducive to improve the patients’quality of life.
