榄香烯同步放化疗治疗局部晚期鼻咽癌患者的临床治疗效果观察

摘要

目的 观察适型调强放疗同步PF方案化疗治疗局部晚期鼻咽癌的过程中联合应用榄香烯的临床疗效、毒性不良反应及生活质量.方法 48例确诊为局部晚期鼻咽癌患者,随机分为对照组及观察组,每组各24例.两组患者均接受适型调强放疗同步PF方案化疗的综合治疗,放疗采用适型调强放疗IMRT技术,放疗过程中及放疗后共行6周期PF方案化疗,21天为一个周期.观察组在每周期化疗过程中联合应用榄香烯注射液,每天1次静脉滴注,10天为一个周期.治疗期间每周行相关化验,每周记录及评估相关不良反应.6周期化疗结束后1个月评价48例患者近期临床疗效、毒性不良反应及生活质量改善情况.结果 48例患者均完成所有治疗,对照组和观察组近期疾病完全缓解率分别为37.50％和54.17％ (x2=1.343,P＞0.05),有效缓解率分别为79.17％和87.50％(r=0.150,P＞0.05),观察组完全缓解率及有效缓解率均高于对照组,但差异无统计学意义(P＞0.05).毒性不良反应方面,观察组在血液学毒性及消化道反应方面明显低于对照组,差异有统计学意义(P＞0.05),且经治疗后观察组生活质量改善情况好于对照组.结论 榄香烯联合同步放化疗的方案治疗局部晚期鼻咽癌,可降低患者的毒性不良反应,提高生活质量,且不影响疗效,可在临床中进一步推广应用.%Objective To observe the clinical curative effects and adverse effects of elemene combined with concurrent chemoradiotherapy for locally advanced nasopharyngeal carcinoma.Methods Totally 48 patients with locally advanced nasopharyngeal carcinoma were randomly divided into the control group and the observation group,24 cases in each group.All the patients in the two groups received concurrent chenotherapy and IMRT radiotherapy.Both groups received six cycles of PF regimen chemotherapy,with every 21 days as one cycle.Elemene was injected into the patients of observation group during chemotherapy,once a day,ten days a chemotherapy cycle.During chemoradiotherapy,the blood routine and adverse effects were observed and recorded every week.Clinical curative effects and quality of life score were evaluated one months later after six cycles of PF regimen chemotherapy.Results All 48 patients completed the whole treatments.The CR rates in the control and observation group were 37.50％ and 54.17％ (x2 =1.343,P ＞0.05).The RR rates in the control and observation group were 79.17％ and 87.50％ (x2 =0.150,P ＞ 0.05).There way no statistical difference between them.As to adverse effects,the incidence rates of hematology toxicity and gastrointestinal reaction in the observation group were obviously lower than those in the control group.There was a significant statistical difference between them.Also,the quality of life score in the observation group was obviously higher than that in the control group.Conclusion Elemene combined with concurrent chemoradiotherapy for locally advanced nasopharyngeal carcinoma can reduce the adverse effects and increase the quality of life,also it does not affect curative effects,so it is worth to be popularized in clinical treatment.
