P53蛋白过表达和P53基因突变与胶质母细胞瘤预后关系的Meta分析

摘要

目的 胶质母细胞瘤(glioblastoma,GBM)是常见的颅内恶性肿瘤,预后不良.寻找与胶质母细胞瘤预后的相关因素对提高患者生存率有重大意义.P53异常是与肿瘤发生发展有关的重要因素.文中探讨P53蛋白过表达和P53基因突变对GBM预后的影响.方法 应用Meta分析对有关P53蛋白过表达和P53基因突变与GBM预后相关的文献进行综合分析.结果 纳入的文献中研究P53蛋白过表达与GBM预后关系的文献6篇,累计病例539例,阳性率53.4%;P53基因突变与GBM预后关系的文献7篇,累计病例514例,阳性(与预后不良有关)率25.3%.P53过表达和P53突变与GBM预后的合并相对危险度(relative hazard,RH)分别为0.95和0.61,95%可信区间分别为0.85～1.08和0.51～0.73,P值分别为0.829和 0.000.结论 根据所纳入的文献进行Meta分析结果提示P53蛋白过表达与GBM预后无关,而P53基因突变是GBM预后良好的生物学标志物.%Objective Glioblastoma ( GBM) is a common intracranial malignant tumor, and determination of certain prognostic factors ia of great significance in improving the survival rate of the patients. P53 abnormity is an important factor in the development of tumors. The aim of this study was to inveatigate the correlation of P53 overexpression and P53 mutations with the prognosis of CBM. Methods We performed a meta-analysis on the literature relating the correlation of P53 overexpression and P53 mutations with the prognoais of GBM. Results A total of 13 trials were identified, including 6 on P53 overexpression in 539 patients, with a positive rate of 53.4％ , and 7 on P53 mutations in 514 cases, with a positive rate of 25. 3％. The combined relative hazard ( RH) of P53 overexpression on the prognosis of GBM was 0. 98 ( 95％ CI: 0. 78 - 1. 23 . P = 0. 829) . while that of P53 mutations on the prognosis of GBM was 0. 62 (95％ CI: 0. 53 - 0. 73, P = O. OOO). Conclusion The Meta analyse result of the literatures collected showed that P53 mutation is a biomarker for good prognosis of GBM, while P53 overexpression is not correlated with it.
