应用DSC-MR灌注成像鉴别少突胶质肿瘤和星形细胞瘤

         

摘要

目的 分析动态磁敏感对比增强(DSC)MR灌注成像在鉴别少突胶质肿瘤和星形细胞瘤中的作用.方法 回顾性分析84例行DSC-MR灌注成像的WHOⅡ级和Ⅲ级少突胶质肿瘤(n=41)和星形细胞瘤(n=43)患者.选取肿瘤内脑血容量(CBV)和脑血流量(CBF)值最大的区域作为ROIs,并计算出最大相对脑血容量(r CBVmax)和最大相对脑血流量(r CBFmax)值.采用Mann-Whitney U检验比较各组肿瘤的r CBVmax和r CBFmax值,采用ROC曲线分析各值的诊断效能.结果 不同级别少突胶质肿瘤的r CBVmax值均大于相应级别的星形细胞瘤的r CBVmax值,两者间有显著统计学差异(P<0.01).ROC曲线显示出较好的诊断效能.r CBFmax值在各组肿瘤的比较中无统计学差异.结论 DSC-MR灌注成像获取的r CBVmax值在鉴别少突胶质肿瘤和星形细胞瘤中具有一定的应用价值.%Objective To analyze the role of DSC-MR perfusion imaging in the differentiation of oligodendroglial and astrocytic tumors.Methods This study included 84 patients, including WHO grade II and III oligodendroglial (n=41) and astrocytic (n=43) tumors.The maximum cerebral blood volume (CBVmax) and cerebral blood flow (CBFmax) regions of the tumor were selected as ROIs, and the maximum relative cerebral blood volume (rCBVmax) and cerebral blood flow (CBFmax) were calculated.The comparison of rCBVmax and rCBVmax values between different groups were conducted using the Mann-Whitney U test.Receiver operating characteristic (ROC) curve was constructed to determine the diagnostic efficacy of each significant value.Results The r CBVmax of different grade oligodendroglial tumors was significantly higher than that of the corresponding grade astrocytomas (P<0.01).ROC analysis showed high diagnostic efficiency.There was no statistical significance of r CBFmax in differentiating the two groups of tumors.Conclusion The rCBVmax value obtained from DSC-MR perfusion-weighted imaging is useful in differentiating between oligodendroglial and astrocytic tumors.
