前哨淋巴结导航外科技术在胃癌外科治疗中的应用

摘要

Objective:To explore the value of sentinel lymph node navigation surgery in the surgical treatment of gastric cancer.Methods:The patients with early gastric cancer in our hospital from 2013 to 2016 were enrolled in the operation of sentinel lymph node navigation technique.Then analyze:(1) sentinel lymph node navigation surgical technique accuracy, sensitivity and specificity;(2) effects of different pathological parameters on sentinel lymph node navigation.Results:(1) The sentinel lymph node was 80.3% (53/66), the sensitivity was 72.9% (35/48) and the specificity was 100%;(2) the pathologic found that sentinel lymph node positive rate of navigation and the tumor size and depth of invasion is closely related to the sentinel lymph node (P<0.05);the positive rate of independent navigation had no relation to operation mode, the location of the tumor and the degree of differentiation (P>0.05).Conclusion:In the radical gastrectomy, sentinel lymph node navigation surgery can predict the pathological state of regional lymph nodes.%目的:探讨前哨淋巴结导航外科技术在胃癌外科治疗中的价值.方法:收集近2013年~2016年某院早期胃癌的患者,实施术中前哨淋巴结导航技术.分析:(1)前哨淋巴结导航外科技术准确性、敏感性及特异性;(2)不同病理结果参数对前哨淋巴结导航结果的影响.结果:(1)前哨淋巴结对于区域淋巴结准确性是80.3%(53/66),敏感性72.9%(35/48),特异性100%;(2)通过病理学发现,前哨淋巴结导航阳性率与肿瘤大小及浸润深度密切相关(P<0.05);前哨淋巴结导航阳性率与手术方式、肿瘤部位及分化程度无关(P>0.05).结论:在胃癌根治术中,前哨淋巴结导航外科技术可较好的预测区域淋巴结病理状态.
